Konvomep Drug Interactions: What to Avoid and What to Tell Your Doctor

Konvomep (omeprazole and sodium bicarbonate for oral suspension) can interact with a number of other medications in ways that may reduce their effectiveness or increase the risk of serious side effects. Why Does Konvomep Interact With So Many Drugs?

Konvomep's interactions arise from two main sources:

Stomach pH changes. Konvomep significantly raises gastric pH. Many drugs depend on an acidic stomach environment for proper absorption. When stomach acid is reduced, these drugs may not be absorbed correctly, leading to lower blood levels and reduced effectiveness.

CYP enzyme inhibition. Omeprazole inhibits CYP2C19, a liver enzyme that metabolizes many drugs. By slowing the breakdown of these drugs, omeprazole can cause them to build up to higher-than-intended levels.

Contraindicated Combinations (Do Not Use Together)

Rilpivirine (Edurant, Complera, Odefsey, Juluca, Cabenuva): Konvomep is contraindicated with all rilpivirine-containing HIV medications. PPIs dramatically reduce rilpivirine absorption, potentially causing HIV viral load to increase and resistance to develop. If you take a rilpivirine-based HIV regimen, tell your prescriber before starting Konvomep.

Nelfinavir (Viracept): Concomitant use of Konvomep with nelfinavir is contraindicated. Omeprazole significantly reduces nelfinavir serum levels through both CYP2C19 inhibition and reduced gastric absorption.

Major Interactions to Avoid (Talk to Your Doctor)

Clopidogrel (Plavix): This is one of the most clinically significant interactions. Clopidogrel is a blood thinner that must be converted to its active form by CYP2C19. Omeprazole inhibits CYP2C19, reducing clopidogrel's ability to prevent blood clots. This is especially dangerous for people who take clopidogrel after a heart attack or stent placement. If you're on clopidogrel, ask your cardiologist before taking Konvomep.

St. John's Wort: This herbal supplement induces CYP3A4 enzymes, causing the body to break down omeprazole faster than normal. This can significantly reduce omeprazole blood levels and effectiveness. Avoid St. John's Wort while taking Konvomep.

Rifampin (Rifadin): This antibiotic is a potent CYP3A4 inducer. Like St. John's Wort, it dramatically reduces omeprazole levels. Avoid concurrent use with Konvomep.

Moderate Interactions (Monitor Carefully)

Methotrexate: PPIs including Konvomep may increase methotrexate levels, raising the risk of methotrexate toxicity (including kidney damage, mouth sores, and bone marrow suppression). If you take high-dose methotrexate for cancer or autoimmune conditions, your oncologist/rheumatologist should be aware you're taking Konvomep.

Warfarin (Coumadin): Omeprazole can inhibit CYP2C19-mediated metabolism of warfarin, potentially raising INR and increasing bleeding risk. If you're on warfarin, your INR should be monitored more closely when starting or stopping Konvomep.

Tacrolimus (Prograf): Omeprazole may increase tacrolimus serum levels, which can increase the risk of tacrolimus toxicity. Tacrolimus levels should be monitored in transplant patients starting Konvomep.

Voriconazole (Vfend): Voriconazole can increase omeprazole levels, but dosage adjustment of Konvomep is not usually required. However, your doctor should be aware of this combination.

Antifungals (ketoconazole, itraconazole, posaconazole): These drugs depend on acidic gastric conditions for absorption. Konvomep raises stomach pH, potentially reducing levels of these antifungals. Your doctor may need to adjust timing or dosing.

Drug Interactions Related to Sodium Bicarbonate

The sodium bicarbonate component of Konvomep can also affect how other drugs are absorbed:

Tetracycline antibiotics — sodium bicarbonate can reduce absorption of tetracyclines; separate doses by at least 2 hours

Dasatinib (Sprycel) — sodium bicarbonate reduces dasatinib absorption; avoid concurrent use

Drug Interactions With Diagnostic Tests
