Most Critical Interaction: Beta-Blockers and Clonidine Withdrawal
The most dangerous interaction with clonidine involves beta-blockers (metoprolol, atenolol, carvedilol, propranolol, etc.). Under normal circumstances, beta-blockers and clonidine can be used together safely. The danger arises when clonidine is being tapered or discontinued. If a patient abruptly stops clonidine while still on a beta-blocker, the resulting rebound hypertension is more severe — because the beta-blocker blocks the cardiac response that would otherwise partially compensate for the blood pressure spike.
The clinical rule: if a patient on both a beta-blocker and clonidine needs to stop clonidine, the beta-blocker should be tapered and stopped first, then the clonidine is tapered. Never stop clonidine first.

MAO Inhibitors — Contraindicated
Monoamine oxidase inhibitors (MAOIs) — such as phenelzine (Nardil), tranylcypromine (Parnate), and selegiline — are contraindicated with chlorthalidone due to the risk of severe additive hypotensive effects. MAOIs are older antidepressants now rarely used, but it's critical to disclose MAOI use (or recent use within 14 days) to your prescriber.
Tricyclic Antidepressants (TCAs) — Reduce Clonidine's Effectiveness
Tricyclic antidepressants (amitriptyline, nortriptyline, imipramine, desipramine, maprotiline) block the antihypertensive effect of clonidine by inhibiting its uptake into adrenergic neurons. Taking a TCA with clonidine can significantly reduce blood pressure control, potentially leading to poorly managed hypertension. This is a major interaction — your prescriber needs to know about any TCA use, even low doses used for sleep or nerve pain.
Lithium — Risk of Lithium Toxicity
Chlorthalidone (and all thiazide diuretics) reduce the kidney's excretion of lithium by reducing sodium reabsorption, causing lithium to accumulate in the blood. This can lead to lithium toxicity. Lithium has a narrow therapeutic index — even small increases in blood levels can cause toxicity (tremor, nausea, confusion, seizures, cardiac arrhythmias). If you take lithium for bipolar disorder or mood disorders, your prescriber must monitor your lithium levels closely whenever chlorthalidone is started, stopped, or dose-adjusted.
Digoxin — Risk of Toxicity via Hypokalemia
Chlorthalidone lowers potassium levels in the blood. Digoxin (used for heart failure and atrial fibrillation) becomes more toxic when potassium is low. Low potassium allows more digoxin to bind to cardiac cells, increasing the risk of digoxin toxicity — which can cause dangerous heart arrhythmias, nausea, and vision changes. If you take digoxin, regular electrolyte monitoring is essential.
CNS Depressants — Additive Sedation
Clonidine has sedating properties, and combining it with other CNS depressants amplifies this effect. These include:
Alcohol — significantly worsens drowsiness and lowers blood pressure further
Benzodiazepines (Xanax, Valium, Klonopin) — additive sedation
Opioid pain medications (oxycodone, hydrocodone, morphine) — additive CNS depression and enhanced hypotension
Sleep medications (Ambien, Benadryl, others) — additive drowsiness

Diabetes Medications — Blood Sugar Effects
Chlorthalidone can increase blood sugar levels by reducing insulin sensitivity. Additionally, clonidine can mask symptoms of hypoglycemia (tremor, tachycardia, blood pressure changes). Together, this creates a complex interaction for patients on insulin or oral diabetes medications:
