Esgic is a prescription combination medication for tension headaches. Learn what it contains, how it works, dosage instructions, and important safety information for 2026.

Esgic is a prescription combination medication used to treat tension headaches. If your doctor has prescribed it — or if you're wondering whether it might be right for you — this guide covers everything you need to know: what's in it, what it treats, how to take it safely, and what to watch out for.

What Is Esgic?

Esgic is the brand name for a combination of three active ingredients:

  • Butalbital (50mg): A short-to-intermediate-acting barbiturate that relaxes muscles and acts as a sedative by depressing the central nervous system.
  • Acetaminophen (325mg): A non-opioid analgesic and antipyretic that relieves pain by changing how the body senses it.
  • Caffeine (40mg): A central nervous system stimulant that enhances the pain-relieving effects of acetaminophen and may help by constricting cerebral blood vessels.

The brand name Esgic has been discontinued. The medication is now available only as a generic — butalbital/acetaminophen/caffeine — from various manufacturers. Other brand names for the same combination have included Fioricet, Zebutal, Orbivan, Capacet, and Medigesic. The FDA approved this combination in November 1984 for the relief of tension (muscle contraction) headaches.

What Is Esgic Used For?

Esgic is FDA-approved for the treatment of the symptom complex of tension (or muscle contraction) headaches. These are the most common type of headache — the kind that feel like a tight band of pressure around your head, often brought on by stress, poor posture, or muscle tension in the neck and shoulders.

Esgic may also be prescribed off-label for migraine headaches in some patients, though it is not FDA-approved for this use and is generally not considered first-line therapy for migraines. For migraines, triptans are typically preferred.

How Do You Take Esgic?

Esgic (and its generic equivalent) comes in capsule and tablet form. Here are the standard dosing instructions:

  • Standard dose: 1 to 2 tablets or capsules every 4 hours as needed
  • Maximum daily dose: 6 tablets or capsules per day (do not exceed this)
  • Acetaminophen limit: Do not exceed 4,000mg of acetaminophen per day from ALL sources combined (each tablet contains 325mg)
  • Frequency of use: Use as needed, not on a scheduled basis. Limit use to no more than 8-10 days per month to avoid medication-overuse headache.

Esgic can be taken with or without food. If it causes nausea, taking it with food may help. Avoid alcohol while taking Esgic — alcohol increases the risk of liver damage from acetaminophen and enhances the sedative effects of butalbital.

Who Should Not Take Esgic?

Esgic is contraindicated (should not be used) in:

  • People with porphyria (a group of metabolic disorders)
  • People with hypersensitivity (allergy) to butalbital, acetaminophen, caffeine, or any other barbiturates
  • Children under 12 years old (safety and efficacy not established)

Use with caution (and discuss with your doctor) if you have liver disease, kidney disease, a history of drug or alcohol dependence, depression, or are elderly.

Is Esgic a Controlled Substance?

The answer depends on where you live and which specific manufacturer's product your pharmacy dispenses. Butalbital is classified as Schedule III federally. However, many Esgic formulations appear on the DEA's Exempt Prescription Products list, which removes the controlled substance designation for those specific products at the federal level. Critically, many states have enacted their own laws classifying all butalbital products as Schedule III regardless of federal status. Check with your pharmacist or doctor about the rules in your state.

How Much Does Esgic Cost?

Generic butalbital/acetaminophen/caffeine typically costs $45-$55 per 30 tablets at retail without insurance. With a GoodRx coupon, you can pay as little as $14.54. With SingleCare, prices can go as low as $10.73. Most insurance plans cover the generic at Tier 1-2 with a copay of $0-$20. For more details, see our guide to saving money on Esgic.

Frequently Asked Questions

Esgic (butalbital/acetaminophen/caffeine) is FDA-approved for tension (muscle contraction) headaches. It may also be used off-label for migraines in some patients, though triptans are generally preferred for migraines. It is not approved for headache prevention — only acute (as-needed) treatment.

The maximum dose of Esgic is 6 tablets or capsules per day. Additionally, the total acetaminophen from all sources (including other medications) must not exceed 4,000mg per day. Each Esgic tablet contains 325mg of acetaminophen. Exceeding these limits significantly increases the risk of liver damage.

Yes. Esgic and Fioricet are (or were) different brand names for the same combination medication: butalbital 50mg / acetaminophen 325mg / caffeine 40mg. Both brands have been discontinued, and the medication is now available only as a generic from various manufacturers.

Esgic is typically absorbed well when taken orally. Most patients experience headache relief within 30-60 minutes of taking a dose, though individual response varies. Taking it at the onset of a headache — before the pain becomes severe — generally produces faster and more complete relief than waiting.

No — or only with extreme caution. Both Esgic and Tylenol (acetaminophen) contain acetaminophen. Combining them can easily push your daily acetaminophen intake over the 4,000mg limit, increasing the risk of liver damage. Do not take Tylenol or any other acetaminophen-containing product at the same time as Esgic without first consulting your doctor.
