Esgic Boxed Warning: Liver Toxicity

Esgic carries a FDA boxed warning — the most serious type — about liver toxicity from acetaminophen. Taking too much acetaminophen can cause acute liver failure, sometimes requiring liver transplant or resulting in death. This risk is highest when:

  • You take more than 4,000 mg of acetaminophen per day from all sources combined (each Esgic tablet contains 325mg)
  • You drink alcohol regularly while taking Esgic
  • You have pre-existing liver disease
  • You take other medications containing acetaminophen (Tylenol, NyQuil, Vicodin, etc.)

Common Side Effects of Esgic

  • Drowsiness and sedation: Very common due to butalbital's barbiturate effect. Do not drive or operate heavy machinery until you know how Esgic affects you.
  • Dizziness and lightheadedness: Can occur shortly after taking a dose, especially when standing up quickly. Rise slowly from sitting or lying positions.
  • Nausea and vomiting: Taking Esgic with food may help reduce nausea.
  • Dry mouth: A common effect of butalbital's CNS action.
  • Stomach pain and constipation: GI effects are relatively common, especially with regular use.
  • Feeling intoxicated or "floaty": The barbiturate component can produce mild euphoria or a sense of detachment, particularly at higher doses.

Serious Side Effects — Seek Medical Attention

The following side effects are serious and require prompt medical attention:

  • Signs of liver damage: Yellowing skin or eyes (jaundice), dark urine, clay-colored stools, right upper abdominal pain, unusual fatigue, loss of appetite. Seek care immediately.
  • Severe skin reactions (rare): Stevens-Johnson syndrome can occur with acetaminophen, though rarely. Signs include skin reddening, blistering, and peeling. This is a medical emergency.
  • Respiratory depression: Slow or difficult breathing, especially when combined with other CNS depressants (alcohol, opioids, benzodiazepines). This is life-threatening.
  • Allergic reaction: Hives, swelling of face/lips/throat, or difficulty breathing. Stop taking Esgic and call 911 immediately if this occurs.

Dependence, Withdrawal, and Medication-Overuse Headache

One of the most important risks with Esgic is the potential for physical dependence and medication-overuse headache (MOH). Butalbital is habit-forming — with regular use, your body can become physically dependent on it. Stopping suddenly can cause withdrawal symptoms including anxiety, insomnia, tremors, and seizures. Never stop Esgic abruptly if you've been taking it regularly; work with your doctor to taper gradually.

Medication-overuse headache (MOH) — also called rebound headache — occurs when Esgic is used more than 5 days per month. Instead of relieving headaches, the medication starts causing them. Patients develop more frequent headaches that only resolve temporarily with another dose. This cycle is difficult to break and may require supervised medication withdrawal. This is why clinicians recommend limiting Esgic to no more than 8-10 days per month maximum.

Esgic and Pregnancy/Breastfeeding

All three ingredients in Esgic (butalbital, acetaminophen, caffeine) pass into breast milk. Breastfeeding while taking Esgic is not recommended. If taken late in pregnancy, butalbital can cause withdrawal symptoms in the newborn. Tell your doctor immediately if you are pregnant or planning to become pregnant while taking Esgic.
