Like all combination oral contraceptives, Vestura (drospirenone 3 mg / ethinyl estradiol 0.02 mg) can cause side effects ranging from mild and transient to rare but serious. Most people tolerate it well — the majority of side effects occur in the first 1-3 months and then resolve. Here's a complete overview of what to expect.

Boxed Warning: Smoking and Cardiovascular Risk

The FDA requires all combination oral contraceptives to carry a boxed warning about smoking. Cigarette smoking increases the risk of serious cardiovascular events (including blood clots, stroke, and heart attack) when combined with estrogen-containing birth control. This risk increases with age and the number of cigarettes smoked. Women over 35 who smoke should NOT use Vestura or any COC.

Common Side Effects (Occur in 1–10% of Users)

These side effects are generally mild and often improve after the first 2-3 months:

  • Nausea — occurs in about 10% of users; often worse in the first few months. Taking Vestura with food or at bedtime can help.
  • Headache or migraine — commonly reported; usually mild
  • Breast tenderness or pain — typically resolves after the first few cycles
  • Irregular menstrual bleeding (breakthrough bleeding or spotting) — especially in the first 3 months; usually decreases over time
  • Mood changes — including depression, irritability, or emotional lability; reported in 1–10% of users
  • Decreased libido — can occur with hormonal contraceptives
  • Water retention / bloating — often what patients attribute to "weight gain" on the pill; related to estrogen-driven fluid retention
  • Abdominal discomfort or nausea — GI side effects are common early in use

Serious Side Effects — Seek Medical Care Immediately

While rare, Vestura can cause serious complications. Get emergency medical attention for any of the following:

  • Signs of blood clot (VTE/DVT/PE): Sudden leg pain, swelling, warmth; chest pain; difficulty breathing; coughing up blood
  • Signs of stroke: Sudden weakness or numbness in face/arm/leg, trouble speaking, vision changes, severe headache, loss of balance
  • Signs of heart attack: Chest pain, pressure, tightness; pain radiating to arm or jaw; sweating; nausea
  • Signs of retinal thrombosis: Sudden vision loss, double vision, bulging eye
  • Severe abdominal pain — could indicate liver tumor or gallbladder disease (rare)
  • Jaundice (yellowing of the skin/eyes) — stop Vestura and contact your doctor immediately

The VTE Risk with Drospirenone

Vestura contains drospirenone, a progestin that may be associated with a slightly higher risk of venous thromboembolism (VTE) compared to pills containing levonorgestrel or some other progestins. Epidemiologic studies show the VTE risk with drospirenone-containing COCs ranges from no increase to up to 3x higher than with levonorgestrel COCs. Your doctor will assess your personal risk factors before prescribing.

Side Effects Specific to Drospirenone: Potassium

Drospirenone is a spironolactone analogue with antimineralocorticoid activity, which means it can raise potassium levels. If you take medications that also raise potassium — such as ACE inhibitors, ARBs, potassium-sparing diuretics, or NSAIDs taken long-term — your doctor should check your potassium levels during the first month on Vestura. Avoid taking Vestura if you have kidney disease, adrenal insufficiency, or liver disease.

When Should I Call My Doctor?

Contact your healthcare provider (not the emergency room) if you experience:

  • Breakthrough bleeding that lasts more than 3 months or is very heavy
  • Missed periods after taking pills correctly (rule out pregnancy)
  • New or worsening depression or mood changes that affect daily functioning
  • Persistently high blood pressure at home readings
  • Nausea or GI symptoms that persist beyond 3 months and don't improve with food

Frequently Asked Questions

Weight gain is not a consistently proven side effect of drospirenone/ethinyl estradiol. What many patients experience is water retention (bloating) from the estrogen component, not body fat gain. Drospirenone has anti-mineralocorticoid effects that can actually reduce estrogen-related fluid retention somewhat.

Mood changes including depression and emotional lability are reported in 1-10% of users in clinical studies. However, studies on the link between hormonal contraceptives and depression are mixed. If you experience significant mood changes on Vestura, discuss them with your doctor — switching to a different progestin or a non-hormonal method may help.

Most common side effects (nausea, spotting, breast tenderness) occur in the first 1-3 months and then resolve as your body adjusts. If side effects persist beyond 3 months or are severe, contact your healthcare provider.

The absolute risk of VTE from drospirenone-containing COCs is low but higher than baseline. For healthy non-smoking women under 35 without VTE risk factors, the risk is very small. Women with personal or family history of blood clots, or other VTE risk factors, should discuss alternative contraceptive options with their doctor.

Contact your prescriber and describe your symptoms. If you've been switched to a new generic (such as Gianvi or Loryna) and are experiencing new side effects, give it 2-3 months to allow for adjustment. If symptoms persist, ask your doctor about switching to Nikki (most similar inactive ingredients to Vestura) or brand-name Yaz.
