What are the most common Tribenzor side effects? Learn what to expect from olmesartan/amlodipine/HCTZ, which symptoms are serious, and when to contact your healthcare provider.

Like all medications, Tribenzor (olmesartan/amlodipine/hydrochlorothiazide) can cause side effects. Because it contains three active ingredients — each from a different drug class — the side effect profile reflects contributions from all three components. Most side effects are mild and manageable, but some require prompt medical attention.

This article covers the most important things to know about Tribenzor's side effects — what's common and expected, what's serious and requires a call to your doctor, and the key boxed warning about pregnancy.

Boxed Warning: Fetal Toxicity

Do not use Tribenzor if you are pregnant. Tribenzor carries a boxed warning — the FDA's most serious safety designation — for fetal toxicity. The olmesartan component acts on the renin-angiotensin system, and drugs in this class can cause serious injury or death to a developing fetus, particularly when used during the second or third trimester. If you become pregnant while taking Tribenzor, stop the medication and contact your doctor immediately.

Common Side Effects of Tribenzor (Incidence ≥2%)

In clinical trials, these side effects occurred in 2% or more of patients taking Tribenzor 40/10/25 mg:

Dizziness: The most common reason patients discontinued Tribenzor in trials. Often occurs early in treatment or after a dose increase. Can be worse when standing up quickly.

Peripheral edema (swelling of ankles/legs): From the amlodipine component. Usually mild, but tell your doctor if it worsens or is accompanied by unexplained weight gain.

Headache

Fatigue

Nasopharyngitis (runny nose/cold-like symptoms)

Muscle spasms

Nausea

Upper respiratory tract infections

Diarrhea

Urinary tract infections (UTI)

Joint swelling

Serious Side Effects: Call Your Doctor Right Away

The following side effects are less common but potentially serious. Contact your healthcare provider immediately if you experience them:

Severe hypotension (very low blood pressure): Symptoms include fainting, extreme dizziness, pale skin, lightheadedness. More likely in patients who are volume- or salt-depleted, or taking diuretics.

Kidney problems: Reduced urine output, swelling, or signs of worsening kidney function. Impaired renal function was reported in 2.1% of Tribenzor patients in trials — higher than dual-combination rates.

Electrolyte imbalance: Symptoms of low potassium (muscle weakness, irregular heartbeat), low sodium (confusion, fatigue, nausea), or low magnesium. HCTZ can cause these imbalances.

Sprue-like enteropathy: A rare but serious olmesartan side effect involving severe, chronic diarrhea with significant weight loss. Can develop months to years after starting the medication. If you have persistent severe diarrhea, tell your doctor.

Chest pain or worsening angina: Amlodipine can rarely cause or worsen chest pain, particularly in patients with obstructive coronary artery disease. Call 911 if you have severe chest pain.

Acute myopia or angle-closure glaucoma: A rare HCTZ reaction causing sudden blurred vision or eye pain. Seek immediate ophthalmologic evaluation.

Severe allergic reaction: Hives, facial swelling, difficulty breathing. Stop the medication and seek emergency care.

Side Effects and Lab Monitoring

Your doctor will likely order periodic blood tests while you're on Tribenzor to monitor:

Kidney function (creatinine, BUN)

Electrolytes (potassium, sodium, magnesium)

Blood glucose (HCTZ can impair glucose tolerance, which matters for diabetic patients)

Cholesterol/triglycerides (HCTZ can increase both)

Managing Common Side Effects

Dizziness: Rise slowly from sitting or lying positions. Avoid sudden position changes, especially in the morning.

Ankle swelling: Elevate legs when possible. If significant, tell your doctor — dose adjustment may be needed.

Frequent urination: From HCTZ. Take Tribenzor in the morning or early afternoon to minimize nighttime disruption.

Frequently Asked Questions

Dizziness is the most commonly reported side effect of Tribenzor and was the most common reason patients stopped the medication in clinical trials. It often occurs early in treatment or after a dose increase. Rising slowly from seated or lying positions can help reduce dizziness related to blood pressure changes.

Yes. Impaired renal function was reported in 2.1% of patients taking Tribenzor in clinical trials — higher than rates seen with dual-combination therapy. Regular monitoring of kidney function through blood tests (creatinine, BUN) is recommended during treatment. Tribenzor should be avoided in patients with CrCl ≤30 mL/min.

Sprue-like enteropathy is a rare side effect of the olmesartan component that causes severe, chronic diarrhea with significant weight loss. It can develop months to years after starting treatment and can mimic celiac disease. If you develop persistent severe diarrhea on Tribenzor, tell your doctor — discontinuation typically resolves the condition.

No. Tribenzor carries a boxed warning for fetal toxicity. All drugs that act on the renin-angiotensin system — including the olmesartan component in Tribenzor — can cause injury and death to a developing fetus, particularly in the second and third trimesters. Discontinue immediately if pregnancy is detected and contact your prescriber.

Potentially. The HCTZ component can lower potassium (hypokalemia) while the olmesartan component can raise it. The net effect on potassium is variable and depends on diet, kidney function, and other medications. Your doctor should monitor electrolytes periodically. Symptoms of low potassium include muscle weakness, cramping, and irregular heartbeat.
