Onyda XR Side Effects: What to Expect and When to Call Your Doctor

Most Common Side Effects of Onyda XR as Monotherapy

In clinical studies of clonidine extended-release as monotherapy (ADHD treatment without a stimulant), the most common side effects occurring at least 5% more often than placebo were:

Somnolence (sleepiness): The most commonly reported side effect. Taking Onyda XR at bedtime is designed to reduce daytime sleepiness, but some children may still feel groggy in the morning during dose titration.

Low blood pressure (hypotension) and slow heart rate (bradycardia): Onyda XR can cause dose-related drops in blood pressure and heart rate. Watch for dizziness, fainting, extreme tiredness, or pale skin — especially when your child stands up quickly.

Cardiac conduction abnormalities: Clonidine can worsen sinus node dysfunction or cause AV block, particularly in children with existing heart conditions or those taking other cardiac medications.

Allergic reactions: Signs include hives, skin rash, swelling of the face, lips, or tongue. If your child had an allergic reaction to clonidine patches previously, they may be at higher risk with Onyda XR.

Rebound hypertension (from stopping suddenly): Abruptly stopping Onyda XR can cause a dangerous spike in blood pressure. Symptoms include headache, rapid heart rate, nausea, flushing, chest tightness, and anxiety.

The Most Important Safety Rule: Never Stop Abruptly

This is critical and cannot be overstated. If your child's supply runs out or you decide to discontinue treatment, do NOT simply stop. The FDA prescribing information requires tapering the dose in decrements of no more than 0.1 mg every 3 to 7 days. Always talk to your doctor before stopping Onyda XR.
