1. CNS Depressants (Alcohol, Barbiturates, Sedating Antihistamines, Benzodiazepines)

Onyda XR can potentiate (strengthen) the sedating effects of any CNS depressant. Taking Onyda XR with alcohol, barbiturates (like phenobarbital), benzodiazepines (Valium, Xanax), or sedating antihistamines (diphenhydramine/Benadryl, doxylamine) can cause excessive sedation, dangerous drowsiness, and in severe cases, breathing problems.

What to do: Avoid combining with CNS depressants. Tell your doctor if your child takes any sleep aids, anti-anxiety medications, or antihistamines (even over-the-counter ones).

2. Drugs That Affect Sinus Node Function or AV Nodal Conduction

Onyda XR can cause bradycardia (slow heart rate) and affect the heart's electrical conduction. Combining it with drugs that also affect heart rate can create additive effects — potentially causing dangerous bradycardia or AV block. These drugs include:

Digitalis (digoxin): Used for heart failure and arrhythmias

Calcium channel blockers: Diltiazem, verapamil, amlodipine — used for blood pressure and heart conditions

Beta-blockers: Metoprolol, propranolol, atenolol — used for heart conditions, migraines, or anxiety

What to do: Avoid if possible. If required, titrate slowly and monitor vital signs frequently.

Moderate Interactions — Use With Caution and Monitoring

3. Tricyclic Antidepressants (TCAs)

Tricyclic antidepressants (amitriptyline, nortriptyline, imipramine, desipramine) can reduce the blood-pressure-lowering effect of clonidine and may cause blood pressure to increase. Concurrent use can also counteract Onyda XR's therapeutic effects.

What to do: Monitor blood pressure closely if a TCA is required. Discuss with your child's doctor whether the combination is appropriate.

4. Antihypertensive Medications

Any medication that lowers blood pressure — including ACE inhibitors, ARBs, diuretics — will have its blood-pressure-lowering effect potentiated (made stronger) when combined with Onyda XR. This could cause excessive low blood pressure (hypotension) and fainting.

What to do: Use caution. Monitor blood pressure and heart rate regularly. Your doctor may need to adjust doses of one or both medications.

5. Other Clonidine Products

Onyda XR must not be combined with other clonidine products (Kapvay tablets, Catapres immediate-release tablets, or Catapres-TTS patches). Taking two clonidine-containing products simultaneously can cause dangerous blood pressure drops and excessive sedation.

What to do: Stop all other clonidine products before starting Onyda XR, under your doctor's guidance.

Food and Supplement Interactions

High-fat meals: In pharmacokinetic studies, a high-fat meal (approximately 900 calories) did not meaningfully affect the absorption of clonidine from Onyda XR. It can be taken with or without food.

Alcohol: Alcohol potentiates the CNS depressant effects of Onyda XR. Avoid alcohol use in patients taking Onyda XR.
