Boxed Warning: What You Must Know Before Taking Ninjacof XG

Ninjacof XG carries an FDA boxed warning — the most serious type of warning required for prescription medications. Key points:

• Opioid risk: Codeine has a risk for addiction, abuse, and misuse, which can lead to overdose and death.
• Respiratory depression: Serious, life-threatening, and fatal respiratory depression can occur. The risk is highest when starting this medication or taking more than the recommended dose.
• CNS depressant interactions: Taking Ninjacof XG with benzodiazepines, other opioids, alcohol, or other CNS depressants can cause profound sedation, respiratory depression, coma, and death.
• Not for children under 18: Opioid cough suppressants are not recommended for patients under 18 years of age due to the risk of serious or fatal breathing problems.

Common Side Effects (Expected, Generally Mild)

Most people taking Ninjacof XG as prescribed will experience some of these side effects, especially when first starting the medication:

• Drowsiness or sedation — the most common side effect; avoid driving or operating heavy machinery
• Dizziness or lightheadedness — rise slowly from sitting or lying positions to reduce fall risk
• Nausea or vomiting — taking the medication with food can reduce stomach upset
• Constipation — especially with longer use; stay well-hydrated and ask your doctor about a stool softener if needed
• Facial flushing — a feeling of warmth or redness in the face, common with opioid use
• Stomach upset — guaifenesin may cause mild gastrointestinal discomfort in some patients

Serious Side Effects: Call Your Doctor Right Away

Contact your healthcare provider immediately if you experience any of the following:

• Unusual extreme drowsiness or difficulty staying awake
• Agitation, confusion, or unusual changes in mental status
• Withdrawal symptoms if you stop abruptly: restlessness, watery eyes, runny nose, nausea, sweating, muscle aches
• Signs of an allergic reaction: rash, hives, swelling of the face/throat, difficulty breathing

Emergency Warning Signs: Call 911 Immediately

Get emergency help immediately for any of these signs, which may indicate opioid overdose or severe respiratory depression:

• Slow, shallow, or stopped breathing
• Cannot be awakened or won't respond
• Blue or purple lips or fingernails (cyanosis)
• Extreme dizziness, seizures, or cold, clammy skin

If naloxone (Narcan) is available, administer it and call 911. Naloxone reverses opioid overdose and can be lifesaving.

Special Populations and Additional Risks

• Pregnancy: Using codeine during pregnancy may slightly increase birth defect risk and can cause neonatal withdrawal syndrome in newborns. Not recommended during pregnancy.
• Breastfeeding: Codeine can pass into breast milk. Some women metabolize codeine too rapidly (ultra-rapid metabolizers), leading to dangerously high levels in breast milk. Contact your doctor before taking Ninjacof XG if breastfeeding.
• Older adults: Elderly patients are more sensitive to CNS and respiratory depression from opioids. Use the lowest effective dose for the shortest possible time.

• Do not exceed the recommended dose — higher doses significantly increase the risk of serious side effects
• Avoid alcohol and other CNS depressants while taking Ninjacof XG
• Use only for the shortest time necessary — opioid cough medications are intended for short-term use (typically no more than 5–7 days)
