Why Does Integra F Interact With Other Medications?

The primary interaction mechanism is chelation — iron binds physically to other drugs in the digestive tract, forming insoluble complexes that can't be absorbed. The result is that the other medication is excreted rather than absorbed, reducing its effectiveness. These interactions occur in the GI tract and can be managed by timing doses apart.

Major Interactions: Separate These by at Least 2 Hours

  • Tetracycline antibiotics (tetracycline, doxycycline, minocycline): Iron significantly reduces tetracycline absorption — by as much as 40–90%. Take Integra F at least 2 hours before or 2 hours after tetracyclines. This is a clinically significant interaction that can reduce antibiotic effectiveness.
  • Fluoroquinolone antibiotics (ciprofloxacin, levofloxacin, ofloxacin): Iron chelates fluoroquinolones in the gut, substantially reducing their absorption. Separate doses by at least 2 hours (take iron 2 hours before or 4–6 hours after the antibiotic for maximum separation).
  • Levothyroxine (Synthroid, Levoxyl): This is a critical interaction for thyroid patients. Iron significantly reduces levothyroxine absorption, potentially causing hypothyroid symptoms or requiring dose adjustments. Separate by at least 4 hours — take levothyroxine in the morning and Integra F at a different time of day.

Moderate Interactions: Use Caution and Separate Doses

  • Antacids (Tums, Maalox, Mylanta): Antacids raise stomach pH and bind to iron, reducing its absorption. Avoid taking antacids within 2 hours of Integra F.
  • Proton pump inhibitors (omeprazole, pantoprazole, esomeprazole): PPIs reduce stomach acid, which impairs the conversion of ferric iron to ferrous iron and decreases overall iron absorption. If you take a PPI, ask your doctor whether iron absorption is adequate on your current regimen.
  • Calcium supplements: Calcium competes with iron for intestinal absorption transporters. Take calcium supplements and Integra F at least 2 hours apart.
  • Levodopa / carbidopa (Sinemet): Iron chelates levodopa, reducing its absorption and potentially reducing effectiveness of Parkinson's disease treatment. Separate doses by at least 2 hours.
  • Bisphosphonates (alendronate/Fosamax, risedronate): Iron can reduce bisphosphonate absorption. Take bisphosphonates on an empty stomach, well apart from iron.

Food and Supplement Interactions

  • Dairy products (milk, yogurt, cheese): Calcium in dairy can impair iron absorption. Do not take Integra F with dairy foods.
  • Coffee and tea: Polyphenols and tannins in coffee and tea inhibit iron absorption. Avoid taking Integra F within 1 hour of coffee or tea.
  • Alcohol: Combining alcohol with the niacin component of Integra F may cause facial flushing. Excessive alcohol use should be avoided, especially due to potential hepatotoxicity when combined with niacin.

Special Interaction: Folic Acid and Pernicious Anemia

Folic acid (1 mg in Integra F) can mask the blood-test signs of vitamin B12 deficiency (pernicious anemia). If you have pernicious anemia or are at risk for it (elderly patients, vegans, those with autoimmune conditions), taking Integra F could allow neurological damage from B12 deficiency to progress undetected while hematological parameters appear normal. Tell your doctor if B12 deficiency is a concern.
