Foundayo Drug Interactions: What to Avoid and What to Tell Your Doctor

Foundayo (orforglipron) is metabolized primarily through the CYP3A4 enzyme pathway in the liver. This means certain medications can either increase or decrease Foundayo's levels in your body — or conversely, Foundayo can affect how other drugs work. Understanding these interactions before you start is important for both safety and effectiveness.

Always Tell Your Doctor About These Drug Categories

Before starting Foundayo, make sure your prescriber knows about ALL medications you take — prescription, over-the-counter, vitamins, supplements, and herbal products. The categories most relevant to Foundayo interactions include:

  • HIV medications (especially ritonavir, cobicistat-boosted regimens)
  • Antibiotics (especially clarithromycin and erythromycin)
  • Antifungals (especially ketoconazole, itraconazole)

Strong CYP3A4 inhibitors that also inhibit OATP1B transporters can significantly increase orforglipron's concentration in your blood. This increases both the intended effects and the risk of side effects.

  • Ritonavir (HIV medication): Avoid concomitant use, or if unavoidable, cap Foundayo at 9 mg/day maximum. Ritonavir is both a strong CYP3A4 inhibitor and OATP1B inhibitor.
  • Clarithromycin (antibiotic): Mentioned in WebMD's Foundayo interaction list as a strong CYP3A4 inhibitor that may increase Foundayo levels.
  • Cyclosporine (immunosuppressant): CYP3A4 inhibitor; may increase Foundayo levels. Discuss management with your provider.

Interactions That Decrease Foundayo Levels (Reduced Effectiveness)

Strong CYP3A4 inducers speed up the breakdown of Foundayo in your liver, meaning less drug reaches your bloodstream. This reduces its effectiveness for weight loss.

  • Carbamazepine (Tegretol) — seizure/nerve pain medication: Avoid concomitant use. If you need carbamazepine, discuss alternative seizure treatments or epilepsy management with your neurologist.
  • Rifampin (antibiotic for TB and certain other infections): Strong CYP3A4 inducer; avoid concurrent use with Foundayo.
  • Moderate CYP3A4 inducers: Your doctor should monitor Foundayo effectiveness and may need to escalate your dose as needed.

How Foundayo Affects Other Medications

Because Foundayo slows gastric emptying, it can affect how quickly other oral medications are absorbed. Additionally, Foundayo itself can increase levels of certain co-administered drugs:

  • Simvastatin: Foundayo significantly increases simvastatin acid levels, raising the risk of muscle side effects (myopathy). Limit simvastatin to 20 mg/day maximum when taking Foundayo. Rosuvastatin and atorvastatin are not significantly affected and are preferred statin options.
  • Oral contraceptive pills: Foundayo delays stomach emptying, which can reduce the absorption of oral birth control pills, potentially making them less effective. Women of childbearing age should switch to a non-oral contraceptive method or add a barrier method for 30 days after starting Foundayo and for 30 days after each dose increase.
  • Other oral medications generally: For medications with a narrow therapeutic window (e.g., warfarin, some heart medications, levothyroxine), the gastric emptying delay may affect absorption timing. Discuss timing and monitoring with your healthcare provider.

The Most Important "Never Combine" Interaction

Never take Foundayo with another GLP-1 receptor agonist. Combining Foundayo with any other GLP-1 drug (Ozempic, Wegovy, Victoza, Trulicity, Rybelsus, Zepbound/Mounjaro's GLP-1 component, or any other) dramatically increases the risk of serious GI adverse reactions and is not clinically indicated. This is a hard contraindication in Foundayo's prescribing information.

Foods and Alcohol

There are no known interactions between Foundayo and specific foods. Foundayo can be taken with or without food. The effect of alcohol on Foundayo is unknown — as with most medications, consuming alcohol in excess while on Foundayo is generally not recommended, especially given the GI side effects.
