Focalin XR can increase blood levels of Coumarin-type blood thinners like Warfarin (Coumadin). This raises the risk of bleeding. If you take Warfarin, your doctor may need to check your INR levels more frequently when starting or adjusting Focalin XR.

Because Focalin XR can raise blood pressure and heart rate, it may reduce the effectiveness of antihypertensive medications. If you take blood pressure medication, your doctor should monitor your readings regularly after starting Focalin XR.
Focalin XR's extended-release mechanism relies partly on pH levels in your digestive system. Antacids, proton pump inhibitors (PPIs) like Omeprazole (Prilosec) and Esomeprazole (Nexium), and H2 blockers like Famotidine (Pepcid) may alter how the medication is released. This could lead to too much medication being released at once or the drug not lasting as long as intended.
If you're having surgery, tell your anesthesiologist that you take Focalin XR. Halogenated anesthetics (used during general anesthesia) combined with stimulants can cause a sudden and dangerous increase in blood pressure.
Medications like Clonidine (Catapres) and Guanfacine (Intuniv) are sometimes prescribed alongside ADHD stimulants. While this combination is used in clinical practice, it can have additive effects on heart rate and blood pressure and requires careful monitoring.
