Dutasteride/Tamsulosin Side Effects: What to Expect and When to Call Your Doctor

Dutasteride/tamsulosin (Jalyn) is a combination BPH medication that is generally well-tolerated — but like all medications, it comes with a profile of potential side effects. Some are common and manageable; others are rare but serious. Common Side Effects (Reported in ≥1% of Patients)

In clinical trials, the most commonly reported side effects of the dutasteride/tamsulosin combination included:

Ejaculation disorders (delayed ejaculation, reduced ejaculate volume, or retrograde ejaculation) — The most common sexual side effect, occurring in about 7.6% of patients early in treatment. This is more common with the combination than with either drug alone.

Impotence / erectile dysfunction — Reported in roughly 9% of patients. Often improves over time or after stopping the dutasteride component.

Decreased libido — Reduced sex drive, more common in the first year of treatment.

Breast disorders (gynecomastia — breast enlargement and/or tenderness) — Due to altered androgen/estrogen balance from dutasteride's DHT-lowering effect.

Dizziness — Primarily from tamsulosin's blood-pressure-lowering effect. Most common when first starting or after a dose change.

Runny or stuffy nose (rhinitis) — A known tamsulosin side effect due to alpha-blocker effects on nasal blood vessels.

Serious Side Effects: When to Get Help Right Away

1. Low Blood Pressure When Standing (Orthostatic Hypotension)

Tamsulosin can cause a sudden drop in blood pressure when you stand up from a sitting or lying position. Symptoms include dizziness, lightheadedness, or fainting. This is most likely:

After your first dose or after a dose change

If you take blood pressure medications, PDE5 inhibitors (e.g., sildenafil, tadalafil), or other alpha-blockers simultaneously

If you become dehydrated or stand quickly in hot weather

What to do: Rise slowly from seated or lying positions. If you feel faint, sit or lie back down immediately. If fainting occurs or symptoms are severe, call your doctor.

2. Priapism (Persistent, Painful Erection)

Priapism — a prolonged, painful erection unrelated to sexual activity — is a rare side effect of tamsulosin (estimated at less than 1 in 50,000 patients). If an erection lasts more than 4 hours, get emergency medical care immediately. Priapism can cause permanent erectile dysfunction if not treated promptly.

3. Eye Problems During Surgery (Intraoperative Floppy Iris Syndrome)

Tamsulosin can cause a condition called Intraoperative Floppy Iris Syndrome (IFIS) during cataract or glaucoma surgery, even if you stopped taking the medication before surgery. IFIS can increase the risk of surgical complications. Always tell your ophthalmologist or surgeon that you are taking (or have taken) dutasteride/tamsulosin before any eye surgery.

4. Increased Risk of High-Grade Prostate Cancer

Dutasteride may slightly increase the risk of high-grade (Gleason score 8–10) prostate cancer, though the overall risk of prostate cancer diagnosis is reduced. Jalyn is not approved for prostate cancer prevention. Regular prostate cancer monitoring (including adjusted PSA interpretation — double your observed PSA value while on a 5-ARI) is important. Report any new symptoms such as bone pain, blood in urine, or significant worsening of urinary symptoms to your doctor.

5. Serious Allergic Reactions

Although rare, serious allergic reactions to dutasteride/tamsulosin can occur. Get emergency help immediately if you experience:

Hives, rash, or severe itching

Swelling of the face, tongue, lips, or throat

Difficulty breathing or wheezing

Note: Patients with a sulfa drug allergy (e.g., to Bactrim/sulfamethoxazole, sulfasalazine, or sumatriptan) should inform their doctor before taking tamsulosin — although rare, cross-reactivity has been reported.

Special Precautions

Blood donation: Do not donate blood while taking dutasteride/tamsulosin. Wait at least 6 months after your last dose.
