Pantoprazole (Protonix) is a proton pump inhibitor used for GERD, erosive esophagitis, and Zollinger-Ellison syndrome. Here's everything patients need to know.

Pantoprazole is one of the most prescribed medications in the United States. If your doctor has prescribed it — or you're wondering whether it might help your symptoms — here's a clear, patient-friendly guide to everything you need to know about pantoprazole in 2026.

What Is Pantoprazole?

Pantoprazole is a proton pump inhibitor (PPI) — a type of medication that reduces the amount of acid your stomach produces. It belongs to the same drug class as omeprazole (Prilosec), esomeprazole (Nexium), and lansoprazole (Prevacid), but unlike those drugs, pantoprazole is available by prescription only.

The brand name is Protonix. The generic form, pantoprazole sodium, is manufactured by many companies and is widely available at pharmacies across the US.

What Is Pantoprazole Used For?

Pantoprazole has several FDA-approved uses:

Erosive esophagitis (EE): Damage to the esophagus caused by acid from GERD. Pantoprazole heals this damage and reduces symptom recurrence. It's FDA-approved for adults and children aged 5 years and older with GERD-related EE.

Gastroesophageal reflux disease (GERD): Chronic acid reflux symptoms including heartburn, regurgitation, and discomfort. Pantoprazole is commonly prescribed for both short-term treatment and maintenance.

Zollinger-Ellison Syndrome (ZE Syndrome): A rare condition where tumors in the pancreas or small intestine cause excessive acid production. Pantoprazole is FDA-approved for long-term management, with doses sometimes reaching 240 mg daily.

Pantoprazole is also used off-label for H. pylori eradication (as part of combination antibiotic regimens), stress ulcer prophylaxis in hospitalized patients, and non-ulcer dyspepsia.

Pantoprazole Dosage Forms Available in 2026

Pantoprazole is available in three dosage forms:

Delayed-release tablets: 20 mg and 40 mg. The most commonly dispensed form; taken orally, swallowed whole.

Delayed-release oral suspension granules: 40 mg per packet; mixed with apple juice or applesauce; used for patients who can't swallow tablets.

Intravenous (IV) solution: Used only in hospital settings for patients unable to take oral medications; 40 mg vials and pre-made IV solutions.

How to Take Pantoprazole Correctly

Tablets: Swallow whole — do not crush, split, or chew. May be taken with or without food.

Oral suspension: Take 30 minutes before a meal. Mix only with apple juice (for oral administration) or applesauce (sprinkled on top). Never mix with water, other liquids, or foods — these affect the enteric coating on the granules.

Missed dose: Take as soon as you remember. If it's almost time for your next dose, skip the missed dose. Never double up.

Typical Pantoprazole Dosages

Erosive esophagitis (adults): 40 mg once daily for up to 8 weeks; an additional 8-week course may be considered if not fully healed.

Maintenance of EE healing: 40 mg once daily.

Zollinger-Ellison Syndrome: Starting dose 40 mg twice daily; titrated based on acid output. Some patients require doses up to 240 mg/day.

Children 5–16 years (≥40 kg): 40 mg once daily for up to 8 weeks.

Children 5–16 years (15–39 kg): 20 mg once daily for up to 8 weeks.

Important Things Pantoprazole Does NOT Do

It does not provide immediate relief of heartburn symptoms — antacids or H2 blockers work faster for acute episodes.

It does not treat the underlying cause of GERD — lifestyle and dietary changes are still important.

A good response to pantoprazole does not rule out gastric cancer — if symptoms recur quickly after stopping, follow up with your doctor for further evaluation.

Is Pantoprazole Safe for Long-Term Use?

Pantoprazole is approved for long-term maintenance therapy in certain conditions. However, long-term use carries risks including vitamin B-12 deficiency, low magnesium, bone fractures, and C. difficile infection. See our detailed post on pantoprazole side effects for more. Frequently Asked Questions

Pantoprazole (Protonix) is FDA-approved for the treatment of erosive esophagitis associated with GERD in adults and children 5 and older, maintenance of healed erosive esophagitis, and treatment of pathological hypersecretory conditions including Zollinger-Ellison syndrome. It is also used off-label for H. pylori eradication and stress ulcer prophylaxis.

Pantoprazole delayed-release tablets may be taken with or without food — food does not significantly affect absorption. Pantoprazole oral suspension (granules) should be taken 30 minutes before a meal, mixed only with apple juice or applesauce. Do not mix granules with water or other liquids.

Pantoprazole begins reducing acid secretion within 1–2 hours of the first dose, but maximum acid suppression may take 2–4 days of daily dosing. For healing of erosive esophagitis, most patients see improvement within 4 weeks, with full healing occurring in up to 8 weeks for most patients.

Protonix is the brand name for pantoprazole. The active ingredient is identical — pantoprazole sodium. Generic pantoprazole is FDA-approved as bioequivalent to Protonix and is significantly less expensive, typically $9–$15 with a coupon versus $780+ for brand Protonix.
