What are the most common Veltassa side effects, and which ones are serious? Learn what to expect when starting patiromer and when to contact your doctor.

Starting a new medication always comes with questions about side effects. Veltassa (patiromer) is generally well-tolerated, but like all medications, it can cause side effects in some patients. Knowing what to look out for — and what's actually serious — helps you manage your treatment confidently.

How Safe Is Veltassa?

Veltassa is a non-absorbed medication — it works entirely in your intestines and doesn't enter your bloodstream. This means it doesn't cause the systemic side effects you might expect from a drug that circulates through your body. It also means it's considered safe to use during pregnancy and breastfeeding (it's not absorbed by the mother and doesn't reach the baby). Despite this favorable profile, it does have some GI effects and one important lab abnormality to monitor.

Common Side Effects (Experienced by 2% or More of Patients)

In clinical trials, the most commonly reported adverse reactions were:

Constipation — the most common GI side effect. Usually mild and tends to resolve during the course of treatment. Staying well-hydrated and eating adequate fiber can help. If constipation becomes severe, contact your doctor — Veltassa is contraindicated in patients with bowel obstruction.

Hypomagnesemia (low magnesium) — reported in 5.3% of patients in clinical trials. Patiromer binds magnesium along with potassium in the colon. Your doctor will monitor your magnesium levels and may recommend a magnesium supplement if levels drop.

Diarrhea — can occur in some patients, often alongside or alternating with constipation.

Nausea — generally mild. Taking Veltassa with food may help reduce nausea in some patients.

Abdominal discomfort — includes mild cramping, bloating, or general abdominal pain. Usually manageable and resolves over time.

Flatulence — increased gas is a common and generally benign side effect that often improves with time.

Serious Side Effects: When to Call Your Doctor Right Away

Signs of severe hypomagnesemia: Low magnesium can become dangerous. Call your doctor immediately if you experience dizziness, irregular heartbeats, feeling jittery, muscle cramps or spasms, or a choking/coughing sensation. Severe hypomagnesemia can cause cardiac arrhythmias.

Severe allergic reaction: Get emergency help immediately if you develop hives, difficulty breathing, or swelling of the face, lips, tongue, or throat. Veltassa has caused allergic reactions including lip edema in some patients.

Severe constipation or signs of bowel obstruction: If you experience severe abdominal pain, inability to pass stool, or vomiting — stop taking Veltassa and contact your doctor immediately. Veltassa is contraindicated in bowel obstruction and can worsen GI motility disorders.

Hypokalemia (too-low potassium): Taking too much Veltassa or having your dose titrated too aggressively can lower potassium too much. Symptoms of low potassium include muscle weakness, leg cramps, fatigue, and heart palpitations. Your doctor monitors your potassium levels regularly to prevent this.

Side Effects That Led Patients to Stop Veltassa in Trials

In clinical trials, 2.7% of patients discontinued Veltassa due to GI side effects. Vomiting (0.8%) and diarrhea (0.6%) were the most common reasons. Most adverse reactions were mild to moderate in severity. This relatively low discontinuation rate suggests that most patients tolerate Veltassa well.

Tips for Minimizing Veltassa Side Effects

Stay hydrated — drinking plenty of water helps with both constipation and overall GI comfort

Take Veltassa with or without food — food doesn't significantly affect its effectiveness

Do NOT mix with hot foods or liquids — heat can affect patiromer's binding capacity

Report persistent side effects to your doctor — dose adjustments may reduce GI discomfort

Get regular lab work — your doctor will check both potassium and magnesium at follow-up visits

The Bottom Line on Veltassa Side Effects

Veltassa is generally well-tolerated, with most side effects being GI-related and mild to moderate. The most important thing to watch for is hypomagnesemia — low magnesium — which requires regular lab monitoring and sometimes supplementation. Report any new or worsening symptoms to your doctor promptly. For more on drug interactions that can compound side effects, see our guide on Veltassa drug interactions.

Frequently Asked Questions

The most common side effects of Veltassa (patiromer) include constipation, hypomagnesemia (low magnesium), diarrhea, nausea, abdominal discomfort, and flatulence. Most are mild to moderate and often improve with time or dose adjustment.

Yes. Patiromer binds magnesium in the colon along with potassium, which can lower serum magnesium levels (hypomagnesemia). This was reported in 5.3% of patients in clinical trials. Your doctor will monitor your magnesium levels and may recommend magnesium supplements if levels become too low.

Call your doctor immediately if you experience dizziness, irregular heartbeat, muscle cramps or spasms, severe constipation, or abdominal pain. Seek emergency care if you develop signs of a severe allergic reaction: hives, difficulty breathing, or swelling of the face, lips, or throat.

Yes, constipation is the most common GI side effect of Veltassa and occurs in more than 2% of patients. It is usually mild and tends to resolve during the course of treatment. Staying hydrated and maintaining dietary fiber can help. If constipation is severe, contact your doctor immediately.
