How Twynsta's Two Ingredients Cause Side Effects

Common Side Effects of Twynsta

These side effects are reported frequently and are generally not dangerous, but they can be bothersome:

Peripheral edema (ankle/leg swelling): The most common side effect of the amlodipine component. It occurs because calcium channel blockers dilate blood vessels, allowing fluid to leak into tissues. Often improves over time. Interestingly, combining amlodipine with an ARB like telmisartan actually reduces edema compared to amlodipine alone.

Dizziness: Particularly common when first starting the medication or after a dose increase. Can also occur when standing up quickly (orthostatic hypotension). Usually improves as your body adjusts.

Upper respiratory infection symptoms: Runny nose, sore throat. Note: ARBs like telmisartan are much less likely to cause a dry cough than ACE inhibitors (like lisinopril), which is one reason many patients prefer ARBs.

Serious Side Effects — Call Your Doctor Right Away

The following side effects are less common but can be medically serious. Contact your doctor promptly or seek emergency care:

Fetal toxicity: Twynsta is contraindicated during pregnancy. If you become pregnant while taking Twynsta, stop immediately and call your doctor. The ARB component can cause fetal injury or death if taken during the second or third trimester.

Hyperkalemia (high potassium): ARBs can raise potassium levels. Symptoms include muscle weakness, irregular heartbeat, or numbness. More likely in patients with kidney disease or those taking potassium-sparing diuretics or supplements.

Kidney impairment: Symptoms include decreased urination, unusual swelling, or fatigue. ARBs affect blood flow to the kidneys, which can be a problem in patients with certain kidney conditions or severe dehydration.

Severe hypotension: Very low blood pressure causing fainting, lightheadedness, or loss of consciousness. Risk is higher in volume-depleted or dehydrated patients.

Angioedema: Rapid swelling of the face, lips, tongue, or throat — this is a medical emergency requiring 911. Less common with ARBs than with ACE inhibitors, but it can still occur.

Worsening chest pain (in patients with coronary artery disease): When first starting amlodipine or increasing the dose, patients with severe coronary artery disease may rarely experience increased angina or even a heart attack. Call 911 for any chest pain.

Rhabdomyolysis (rare): Muscle breakdown leading to kidney failure. Symptoms include severe unexplained muscle pain, weakness, and dark-colored (tea or cola-colored) urine. Seek emergency care immediately.

Managing Common Side Effects

If you're experiencing ankle swelling, try elevating your legs when seated or lying down. If dizziness is a problem, rise slowly from sitting or lying positions. Stay well hydrated, especially in hot weather.

If side effects are bothersome, your doctor may be able to reduce the dose of the component causing the problem. Because Twynsta combines two drugs, it's sometimes possible to lower the amlodipine dose while keeping the telmisartan dose the same, reducing edema while maintaining blood pressure control.
