Boxed Warning: Hypersensitivity Reaction (Abacavir)

Triumeq carries the FDA's most serious warning — a boxed warning — about hypersensitivity reactions to abacavir. These reactions are serious and can be fatal if Triumeq is restarted after being stopped.

Symptoms of abacavir hypersensitivity reaction include:

Fever

Skin rash

Nausea, vomiting, diarrhea, or stomach pain

General feeling of being unwell, fatigue, or muscle aches

Shortness of breath, cough, or sore throat

What to do: If you develop any combination of these symptoms, call your doctor immediately. Never restart Triumeq or any other abacavir-containing medication after a suspected hypersensitivity reaction — restarting can cause a severe, life-threatening reaction within hours. This is why HLA-B*5701 testing is mandatory before starting Triumeq. Patients who test positive for this gene variant should never take abacavir.

Boxed Warning: Lactic Acidosis and Hepatomegaly

The NRTI components of Triumeq (abacavir and lamivudine) carry a boxed warning about lactic acidosis — a dangerous buildup of lactic acid in the blood — and severe hepatomegaly (enlarged liver) with steatosis (fatty liver). These conditions have been fatal in some cases. Women and people with obesity may have a higher risk.

Warning signs of lactic acidosis include: unusual muscle pain, weakness, difficulty breathing, stomach pain, nausea, or cold or blue hands and feet. Seek emergency medical care if these occur.

Boxed Warning: Hepatitis B Exacerbation

If you have hepatitis B (HBV) and HIV, stopping Triumeq can cause a sudden severe worsening of hepatitis B. Patients with HBV/HIV co-infection should be closely monitored by their doctor if Triumeq is ever discontinued, and anti-HBV treatment may be needed. This is why it's critical that your provider knows your HBV status before starting and stopping Triumeq.

Common Side Effects of Triumeq

The most commonly reported side effects with Triumeq include:

Headache

Fatigue or tiredness

Insomnia or sleep disturbances

Nausea

Dizziness

Nightmares or vivid dreams (particularly from the dolutegravir component)

These effects often improve or resolve within the first few weeks of treatment. In the SINGLE clinical trial, the most common treatment-emergent adverse reactions (≥2% frequency) with dolutegravir/abacavir/lamivudine were insomnia, headache, and fatigue. Taking Triumeq at night may help manage sleep-related side effects for some patients.

Neuropsychiatric Side Effects

Dolutegravir (one of the three drugs in Triumeq) has been associated with neuropsychiatric effects in some patients. These can include:

Depression, anxiety, or mood changes

Suicidal ideation (rare but reported)

Concentration difficulties or "brain fog"

Weight Gain

Severe abdominal pain, unusual tiredness, or difficulty breathing (possible lactic acidosis)

Yellowing of skin or eyes, dark urine, or severe nausea (liver problems)
