Tolterodine XR Side Effects: What to Expect and When to Call Your Doctor

Tolterodine XR (tolterodine tartrate extended-release capsules, formerly branded as Detrol LA) is generally well tolerated, but like all medications, it can cause side effects. Understanding what to expect — and knowing which symptoms require immediate medical attention — helps you manage your treatment safely and effectively.

How Tolterodine XR Causes Side Effects

Tolterodine XR is an antimuscarinic medication. It works by blocking muscarinic receptors — primarily M3 receptors — in the bladder to reduce urgency and urinary incontinence. However, muscarinic receptors also exist throughout the body: in salivary glands, the digestive tract, the eye, the heart, and the central nervous system. Blocking these receptors outside the bladder is what causes most of tolterodine's side effects, often called "anticholinergic" effects.

Common Side Effects of Tolterodine XR

The following side effects were reported in ≥1% of patients in clinical trials of Detrol LA 4 mg:

Dry mouth — the most common side effect, occurring in about 23% of patients; usually manageable with sips of water or sugar-free gum

Constipation — may improve with increased water and fiber intake

Headache

Dizziness or drowsiness — do not drive or operate machinery until you know how the medication affects you

Blurred vision — avoid driving if this occurs

Abdominal pain or upset stomach

Dry eyes

Important: Tolterodine XR causes significantly less dry mouth than its predecessor, oxybutynin, which is one reason it became a widely preferred OAB treatment. The extended-release formulation also tends to produce fewer side effects than the immediate-release version.

Serious Side Effects — Call Your Doctor or Seek Emergency Care

While serious side effects are uncommon, they do occur. Seek immediate medical attention for any of the following:

Anaphylaxis or angioedema: Swelling of the face, lips, tongue, or throat; difficulty breathing; hives. Stop taking tolterodine immediately and call 911. This can occur with the first dose or later doses.

Urinary retention: Inability to urinate or greatly reduced urination. Contact your doctor immediately — this requires urgent evaluation.

Heart rhythm changes (QT prolongation): Fast or irregular heartbeat, palpitations, dizziness, feeling faint. Seek emergency care. Risk is higher in patients taking other QT-prolonging drugs or CYP3A4 inhibitors.

Confusion, disorientation, memory problems, or hallucinations: These CNS effects are more common in elderly patients. Call your doctor if any of these occur — they may indicate anticholinergic toxicity.

Worsening glaucoma symptoms: Eye pain, vision changes, or seeing halos around lights. Tolterodine XR should be avoided in uncontrolled narrow-angle glaucoma.

Special Considerations for Older Adults

Adults 65 and older are more sensitive to anticholinergic side effects. Tolterodine XR is listed on the American Geriatrics Society Beers Criteria as a medication to use with caution in older adults due to risks including: cognitive impairment, confusion, increased fall risk, constipation, and urinary retention. Tips for Managing Common Side Effects

Dry mouth: Sip water frequently throughout the day, chew sugar-free gum, or use saliva-stimulating lozenges. Avoid salty, acidic, or alcohol-containing products.

Constipation: Increase dietary fiber and water intake. Gentle exercise helps. If severe, ask your doctor about an appropriate stool softener.

Dizziness: Rise from sitting or lying slowly. Avoid alcohol. Do not drive until you know how tolterodine affects your alertness.

Blurred vision: If persistent or worsening, contact your doctor — it could indicate intraocular pressure issues.
