Like all tetracycline antibiotics, Seysara interacts with a number of common substances — from other prescriptions to everyday supplements. Here's a thorough breakdown of what to know.

Contraindicated Combinations: Never Take These With Seysara

These drug combinations are contraindicated with Seysara — meaning they should never be used together:

Oral retinoids (isotretinoin/Accutane, acitretin, bexarotene, tretinoin oral, palovarotene): Combining any retinoid with a tetracycline like Seysara significantly increases the risk of pseudotumor cerebri (intracranial hypertension) — a serious condition where pressure builds up around the brain. Symptoms include severe headaches, visual disturbances, and papilledema. This combination is strictly contraindicated. Never take Seysara if you are currently on isotretinoin or any oral retinoid.

Serious Interactions: Avoid or Use With Extreme Caution

Neuromuscular blocking agents (succinylcholine, atracurium, cisatracurium, pancuronium, vecuronium): Seysara enhances the effects of neuromuscular blocking drugs by pharmacodynamic synergism, increasing the risk of respiratory depression. This is most relevant for patients undergoing surgery or anesthesia — always inform your surgical team if you are taking Seysara.

Cholera vaccine: Do not receive the oral cholera vaccine within 14 days of taking a systemic antibiotic like Seysara. Antibiotics can be active against the live vaccine strain and render the vaccine ineffective.

Digoxin (Lanoxin): Seysara is a P-glycoprotein (P-gp) inhibitor. Coadministration of Seysara 150 mg with digoxin increased digoxin's peak blood level (Cmax) by 26%. Patients on digoxin who start Seysara should be monitored for signs of digoxin toxicity, including nausea, vomiting, vision changes, and irregular heartbeat.

Penicillin antibiotics (amoxicillin, ampicillin, ticarcillin, dicloxacillin, penicillin G): Tetracyclines can interfere with the bactericidal (bacteria-killing) action of penicillins by pharmacodynamic antagonism. If you need a penicillin antibiotic for an infection while taking Seysara, talk to your doctor — they may recommend a different antibiotic class or adjust your treatment plan.

P-gp substrate drugs (afatinib, aliskiren, and others): Since Seysara inhibits the P-gp drug transporter, it can raise blood levels of other drugs that are P-gp substrates. If you take any medications processed by P-gp, your doctor may need to monitor for increased side effects or toxicity.

Supplements and Food That Reduce Seysara Absorption

Several common supplements and foods can bind to sarecycline in your gut, preventing it from being absorbed properly and reducing how well it works. This doesn't mean you can't take these — it means you need to time them away from your Seysara dose:

Antacids (containing aluminum, magnesium, or calcium) and bismuth products: Take Seysara at least 2 hours before or 6 hours after these products

Calcium supplements: Take Seysara at least 2 hours before or 6 hours after calcium

Iron supplements and multivitamins with iron: Take Seysara at least 2 hours before or 4 hours after iron-containing products

Magnesium (including laxatives containing magnesium): Take Seysara at least 2 hours before or 6 hours after magnesium-containing products

Dairy and high-fat meals: A high-fat meal with dairy delayed Seysara's absorption by about 30 minutes and slightly reduced peak levels. While food generally doesn't require avoidance, avoid taking Seysara with a very large, fatty dairy-heavy meal for best absorption.

Interaction with Oral Contraceptives

Unlike older tetracyclines which were historically thought to reduce the effectiveness of oral contraceptives, Seysara actually slightly increases hormone levels from combination oral contraceptives. Clinical data showed that coadministration of Seysara with ethinyl estradiol + norethindrone acetate increased estrogen (EE) Cmax by 14% and AUC by 11%. This is not a reason to stop using oral contraceptives while taking Seysara, but it's something your dermatologist and OB/GYN should know about.
