Contraindicated Combinations: Do Not Take Together

Other carbonic anhydrase inhibitors (acetazolamide, zonisamide, methazolamide): These medications share topiramate's carbonic anhydrase inhibition mechanism. Combining them significantly increases the risk of metabolic acidosis and kidney stone formation. Avoid concurrent use when possible.

Oral contraceptives (birth control pills): Topiramate may reduce the plasma levels of estrogen-containing oral contraceptives, potentially reducing their effectiveness. Patients must use effective contraception during Qsymia therapy (REMS requirement) — if using hormonal birth control, use a non-hormonal backup method or switch to a more reliable method (IUD, implant, barrier + hormonal). Antidiabetic medications (insulin, metformin, sulfonylureas, GLP-1 agonists): Weight loss from Qsymia can improve insulin sensitivity and blood sugar control, which may require dose adjustments in diabetic medications to avoid hypoglycemia. Non-potassium-sparing diuretics (furosemide, thiazides): Both topiramate and potassium-wasting diuretics can lower potassium levels. Combined use increases hypokalemia risk. Monitor potassium levels regularly.

Anticholinergic medications (diphenhydramine, oxybutynin, tricyclic antidepressants, some antihistamines): Increased risk of heat-related disorders (hyperthermia) when combined with topiramate's anhidrosis (decreased sweating) effect.

Lithium: Phentermine may affect lithium levels. Monitor lithium concentrations if combined.

Ketogenic diet: Combining Qsymia with a ketogenic diet increases the risk of kidney stone formation and metabolic acidosis. Discuss dietary approach with your doctor.

Drug-Food Interactions

Alcohol: As noted above — avoid completely.

High fluid intake: Staying well hydrated reduces kidney stone risk from topiramate. Aim for 8+ glasses of water daily.
