Nizatidine Side Effects: What to Expect and When to Call Your Doctor

Nizatidine is generally well-tolerated by most people who take it. Compared to other medications in its class, it has a relatively clean side effect profile and fewer drug interactions than cimetidine (Tagamet). That said, like any medication, nizatidine can cause side effects — and knowing what to expect helps you stay on top of your health.

Common Side Effects of Nizatidine

The following side effects are the most commonly reported with nizatidine. They often improve as your body adjusts to the medication:

Headache: One of the most frequently reported side effects. Usually mild. If persistent or severe, discuss with your doctor.

Diarrhea: Relatively common. Taking nizatidine with food may help reduce GI upset.

Nausea and stomach discomfort: Some patients report mild nausea, especially when starting the medication.

Dizziness: Usually mild and transient. Avoid driving or operating heavy machinery if you feel dizzy.

Runny nose / upper respiratory symptoms: Some patients report nasal symptoms that may or may not be medication-related.

Sweating: More common with nizatidine than with placebo in clinical studies.

Urticaria (hives): Also reported more frequently with nizatidine than placebo; notify your doctor if a skin rash develops.

Serious Side Effects — Call Your Doctor

While rare, the following serious side effects have been reported with nizatidine. Contact your doctor or go to the emergency room if you experience any of these:

Signs of a serious allergic reaction: Hives that spread rapidly, difficulty breathing, swelling of the face, lips, tongue, or throat — seek emergency care immediately

Unusual bruising or bleeding: May suggest thrombocytopenia (low platelet count), a rare but reported reaction. Call your doctor right away.

Signs of liver problems: Yellowing of the skin or eyes (jaundice), dark urine, severe upper abdominal pain. Elevated liver enzymes have been reported in some patients; usually reversible upon stopping the medication.

Fever with rash or joint pain: May indicate a serum sickness-like reaction. Rare but reported. Contact your doctor.

Signs of low blood counts: Unusual fatigue, frequent infections, or pale skin may suggest anemia or low white blood cell count. Rare with nizatidine.

Special Populations: Who Should Be Extra Careful?

Patients with kidney problems:

Nizatidine is primarily excreted by the kidneys. If you have reduced kidney function (CrCl <50 mL/min), nizatidine can accumulate in your body, increasing the risk of side effects. Your doctor should reduce your dose accordingly. People with moderate impairment typically take 150 mg once daily; those with severe impairment take 150 mg every other day.

Elderly patients:

Older adults are more sensitive to nizatidine and are more likely to have age-related kidney decline. Careful dosing and monitoring are important. Discuss with your doctor if you're over 65.

Pregnant and breastfeeding women:

There are no adequate studies on nizatidine use during pregnancy. Do not use without a doctor's guidance. Nizatidine passes into breast milk and may affect a nursing infant — breastfeeding is generally not recommended while taking nizatidine.

Does Nizatidine Affect Lab Tests?

Yes — nizatidine can affect the results of certain lab tests. If you're having blood tests or urine tests, tell your healthcare provider that you're taking nizatidine. This is especially important if you're being tested for gastric cancer markers or liver enzymes, as nizatidine may alter results.

A Note on Masking Serious Conditions

Nizatidine relieves symptoms of acid-related disorders, but it does not cure underlying causes. Symptom relief does not rule out gastric cancer.
