NasalCrom Drug Interactions: What to Avoid and What to Tell Your Doctor

NasalCrom has one of the cleanest interaction profiles of any allergy medication. But there are still important things to tell your doctor and watch for.

One of NasalCrom's most appealing qualities is its remarkably clean drug interaction profile. Because cromolyn sodium is minimally absorbed into the bloodstream (less than 7% of each dose), it has little opportunity to interact with other medications you might be taking. That said, there are still things your doctor and pharmacist should know before you use it.

Does NasalCrom Interact with Other Medications?

According to WebMD, Mayo Clinic, and the FDA-approved labeling for NasalCrom: there are no known clinically significant drug-drug interactions between cromolyn sodium nasal spray and other medications.

This is one of the most favorable interaction profiles of any allergy medication available. It means NasalCrom can generally be used alongside:

  • Oral antihistamines (cetirizine, loratadine, fexofenadine, diphenhydramine)
  • Intranasal corticosteroids (fluticasone/Flonase, triamcinolone/Nasacort, budesonide/Rhinocort)
  • Oral decongestants (pseudoephedrine/Sudafed)
  • Nasal antihistamines (azelastine/Astepro)
  • Leukotriene modifiers (montelukast/Singulair)
  • Most prescription medications (cardiovascular drugs, antidepressants, diabetes medications, thyroid medications, etc.)

Does NasalCrom Interact with Food or Alcohol?

No known food-drug interactions have been identified for NasalCrom. It is also not known to interact with alcohol. Because the medication acts locally in the nasal passages and is minimally absorbed systemically, dietary intake has no effect on its activity.

Can You Use NasalCrom with Flonase?

Yes. NasalCrom and Flonase (fluticasone propionate) can be used at the same time — they work through entirely different mechanisms and don't interact. NasalCrom prevents mast cell degranulation; Flonase reduces nasal inflammation through corticosteroid activity. Using both can be appropriate when NasalCrom alone doesn't fully control symptoms.

Practical tip: If using both, wait at least 15 minutes between each spray to allow the first to absorb properly before applying the second.

What About NasalCrom and Afrin (Oxymetazoline)?

There is no known drug interaction between NasalCrom and Afrin (oxymetazoline). However, Afrin itself should only be used for 3 days at a time to prevent rebound congestion — an issue that doesn't apply to NasalCrom. Using both for more than 3 days is not recommended due to Afrin's limitations, not because of a NasalCrom interaction.

Preservative Sensitivity: A Note on Benzalkonium Chloride

NasalCrom contains benzalkonium chloride (BAC) as a preservative. Some patients who are sensitive to BAC may experience nasal irritation that isn't related to the cromolyn sodium itself. BAC sensitivity is also found in some other nasal sprays. If you notice persistent irritation that worsens over time — rather than the brief post-spray stinging — mention this to your pharmacist or provider. They may recommend switching to a preservative-free formulation if one is available, or trying a different allergy treatment.

What to Tell Your Doctor Before Using NasalCrom

Even though NasalCrom is OTC with few known interactions, your healthcare provider and pharmacist should know if you have:

  • Asthma — NasalCrom is not indicated for asthma; it should not replace asthma inhalers
  • Wheezing or breathing problems before starting
  • Fever or discolored nasal discharge — suggests infection rather than allergy
  • Pregnancy or breastfeeding — low risk due to minimal absorption, but consult your provider
  • Known allergy to cromolyn sodium or benzalkonium chloride

Can NasalCrom Be Used with Other Supplements?

No known interactions between NasalCrom and vitamins, herbal supplements, or dietary products have been identified. Always tell your pharmacist about all supplements you take, as interactions can sometimes occur with medications other than NasalCrom — even if NasalCrom itself is fine.
