A practical guide for OB/GYNs, PCPs, and NPs on helping patients find Norgestrel (Opill) in stock, navigate insurance, and access cost assistance programs.

Since the FDA approved norgestrel (Opill) for OTC sale in July 2023 and it became available on pharmacy shelves in March 2024, providers have found themselves in a new role: counseling patients on how to navigate a retail distribution system rather than a pharmacy benefits system. This guide gives you the practical tools to make that conversation efficient and effective.

The OTC approval of norgestrel is a landmark in reproductive health access. But providers continue to see patients who can't find it, can't afford it, or don't understand how OTC contraception fits into their existing coverage. Here's how to address each scenario.

Scenario 1: The Patient Who Can't Find Opill at Their Pharmacy

Norgestrel is not in shortage, but individual store availability can vary. If a patient reports their local pharmacy is out of stock, direct them to these steps:

  1. Check multiple pharmacy locations. CVS and Walgreens websites allow patients to check OTC product availability by zip code before driving to the store.
  2. Order online. Opill.com offers discreet home delivery with a 5% savings for auto-shipping. Amazon also carries it. A 3-month supply ($49.99) lowers the monthly cost to about $16.67.
  3. Ask the pharmacy to order it. Pharmacies can place special orders for OTC products upon patient request. Most orders arrive within 1–3 business days.

  • Consider switching to norethindrone. If cost remains a barrier, prescribe norethindrone 0.35 mg — the analogous prescription POP. Generic versions are widely available and typically covered at zero cost under ACA mandates.

Scenario 3: The Patient Who Doesn't Know OTC Birth Control Is Available

Many patients don't yet know that Opill exists. They may still be scheduling appointments to renew prescription birth control when norgestrel would work equally well for them — and is available without a visit. Proactively counseling patients during annual visits about OTC contraceptive options is an opportunity to improve access.

Key counseling points:

  • Opill (norgestrel 0.075 mg) is available at pharmacies and online without a prescription, doctor's visit, or age restriction.
  • It must be taken at the same time every day (within a 3-hour window) for maximum effectiveness. Set a daily phone alarm.
  • It is NOT an emergency contraceptive. It does not prevent pregnancy after unprotected sex.
  • It does not protect against STIs. Advise patients to continue using condoms if STI prevention is a concern.
  • Primary contraindication: current or prior breast cancer.

Scenario 4: The Patient Experiencing Side Effects on Norgestrel

The most common side effect of norgestrel is irregular bleeding — including spotting, lighter periods, or missed periods. Counsel patients that this is expected and typically improves within 2–3 months. If irregular bleeding is persistent, heavy, or unacceptable, consider switching to drospirenone (Slynd), which has a more structured 24/4 pill schedule and may provide a more predictable bleeding pattern.

Headaches and nausea are also reported. Advise taking the pill at the same time daily with a small amount of food to minimize GI effects.
