How Does Yaz 28 Prevent Pregnancy?

Combination oral contraceptives like Yaz 28 prevent pregnancy through three complementary mechanisms:

Suppressing ovulation — the combined effect of drospirenone and ethinyl estradiol suppresses the hormonal signals (LH and FSH) from the pituitary gland that normally trigger the ovaries to release an egg. In clinical studies of Yaz 28, more than 90% of subjects demonstrated ovulation inhibition. No egg released = no fertilization possible.

Thickening cervical mucus — the progestin (drospirenone) thickens the mucus at the cervix, making it harder for sperm to penetrate and reach an egg — even if ovulation were to occur.

Why Does the 24/4 Regimen Improve Efficacy?

Most older combination pills use a 21/7 regimen — 21 active pills followed by 7 days of either placebo or no pill. Yaz 28 uses a 24/4 regimen — 24 active pills and only 4 placebo days. This shorter hormone-free interval means your body has less time for hormonal fluctuation between packs. Research shows the 24/4 format provides more consistent ovulation suppression over the hormone-free period, which improves the contraceptive margin of safety if you start your next pack a day or two late.

How Does Yaz 28 Treat PMDD?

How Does Yaz 28 Clear Acne?

Hormonal acne is driven largely by androgens — male sex hormones (like testosterone) that stimulate sebaceous (oil) glands in the skin to produce excess sebum, which can clog pores and cause breakouts. Drospirenone is an anti-androgen: it blocks androgen receptors and reduces the stimulation of oil glands.

Additionally, ethinyl estradiol stimulates the liver to increase production of sex hormone-binding globulin (SHBG), a protein that binds free testosterone in the bloodstream. More SHBG means less free testosterone available to stimulate oil glands — another mechanism that reduces acne.

Why Can Drospirenone Raise Potassium Levels?

Because drospirenone blocks aldosterone activity (via the mineralocorticoid receptor), it can also mimic spironolactone's effect on potassium: less aldosterone activity means the kidneys retain more potassium and excrete less of it. This is why Yaz 28 is contraindicated in patients with kidney, liver, or adrenal disease, and why potassium monitoring is recommended during the first cycle for patients on other medications that also raise potassium.
