Think of bacteria as tiny fortresses surrounded by walls. These cell walls are essential to bacterial survival — without them, the bacteria fall apart and die. Firvanq works by interfering with the construction of these bacterial cell walls.

Why Does It Only Work on Certain Bacteria?
Vancomycin only works against gram-positive bacteria — bacteria that have a thick outer cell wall made of peptidoglycan (the material vancomycin attacks). Gram-negative bacteria have a different outer membrane structure that physically blocks vancomycin from reaching its target. This is why Firvanq is effective against C. diff (Clostridioides difficile) and MRSA (Staphylococcus aureus), both of which are gram-positive, but would not be effective against gram-negative bacteria like E. coli.
Why Does Firvanq Only Work in the Gut (and Not for Other Infections)?
This is one of the most important things to understand about oral vancomycin. When you swallow Firvanq, less than 10% of it is absorbed into your bloodstream. The vast majority stays in the gut — exactly where C. diff bacteria live. This poor absorption actually makes oral vancomycin ideal for intestinal infections: it concentrates where you need it most.
The downside of this poor absorption is that oral Firvanq cannot treat infections elsewhere in the body — lung infections, skin infections, bloodstream infections, or bone infections. For those, you'd need IV (intravenous) vancomycin, which delivers the drug directly into the bloodstream.
How Quickly Does Firvanq Start Working?

By blocking protein synthesis rather than cell wall synthesis, fidaxomicin is active against C. diff at lower drug concentrations and has a narrower spectrum, causing less disruption to the surrounding gut microbiome.
