How Calcium and Fluoride Can Affect Other Drugs

Both calcium and fluoride can affect the absorption of other medications when taken together. Calcium in particular is known to bind to certain drugs in the gastrointestinal tract, reducing their absorption and effectiveness.

Medications That Interact with Florical

• Levothyroxine (Synthroid, Levoxyl) — Calcium carbonate significantly reduces the absorption of thyroid hormone replacement medications. If you take levothyroxine, do not take Florical within 4 hours of your thyroid medication. Take levothyroxine first thing in the morning on an empty stomach, and take Florical with a meal later in the day.
• Fluoroquinolone antibiotics (ciprofloxacin/Cipro, levofloxacin/Levaquin) — Calcium binds to fluoroquinolone antibiotics in the gut, significantly reducing their absorption and potentially making the antibiotic ineffective. Separate Florical from fluoroquinolone antibiotics by at least 2-3 hours (take the antibiotic first, then Florical 2-3 hours later, or vice versa).
• Tetracycline antibiotics (doxycycline, tetracycline, minocycline) — Calcium chelates (binds) tetracycline antibiotics, dramatically reducing their absorption. Avoid taking Florical within 2-3 hours of any tetracycline antibiotic. When on a short course of tetracyclines, consider pausing Florical for the duration of antibiotic treatment and resuming when finished.
• Bisphosphonates (alendronate/Fosamax, risedronate/Actonel) — Calcium reduces the absorption of oral bisphosphonates. Bisphosphonates must be taken on an empty stomach with plain water only. Do not take Florical at the same time as bisphosphonates. Additionally, sodium fluoride (Florical) and bisphosphonates should generally not be taken together — they are alternative approaches to managing bone disease, not combinations. Talk to your doctor.
• Iron supplements — Calcium can inhibit iron absorption. If you take an iron supplement (for anemia, for example), separate it from Florical by at least 2 hours.
• Zinc and magnesium supplements — High-dose calcium can compete with zinc and magnesium for absorption. If you take these minerals, try to separate them from Florical by at least 1-2 hours.

Food and Supplement Interactions

• Dairy and high-calcium foods — Taking Florical with a calcium-rich meal (milk, yogurt, cheese) is not harmful and does not cause dangerous calcium overload at normal doses. However, patients with hypercalcemia risk should be mindful of total daily calcium intake from all sources.
• Other antacids or calcium supplements — If you're also taking Tums (calcium carbonate) as an antacid or other calcium supplements, be careful not to exceed the tolerable upper intake level (UL) for calcium, which is 2,500 mg/day for adults. Adding up all calcium sources is important.
• Vitamin D — Vitamin D enhances calcium absorption. If you're taking a vitamin D supplement alongside Florical, monitor for signs of hypercalcemia (nausea, weakness, frequent urination) if taking high vitamin D doses, as it will increase absorption of the calcium in Florical.

Medical Conditions That Affect Florical Safety

Tell your doctor about these conditions before starting Florical:

• Hypercalcemia — high blood calcium; Florical is contraindicated
• Hyperfluorosis — high fluoride levels; Florical is contraindicated
• Kidney disease — impaired kidneys may not adequately excrete fluoride or calcium; discuss carefully with your doctor
• Arthritis or joint disease — sodium fluoride can aggravate joint pain; monitor closely and stop if arthritis worsens significantly
• Shellfish allergy — the calcium source is oyster shell; discuss alternative calcium supplements with your doctor if you have this allergy
