Learn about Exxua XR side effects, from common issues like dizziness and nausea to serious warnings. Know what to expect and when to contact your doctor.
Starting a new antidepressant comes with questions — and worries about side effects are usually at the top of the list. If your doctor has prescribed Exxua XR (Gepirone) for major depressive disorder, it helps to know what's normal, what's not, and when you should pick up the phone and call your doctor.
This guide covers the common and serious side effects of Exxua XR based on FDA labeling, plus practical tips for managing them.
Exxua XR is a brand-name prescription antidepressant that contains Gepirone, a selective 5-HT1A receptor partial agonist. It belongs to the azapirone class — the same family as Buspirone (Buspar), which is commonly used for anxiety. Exxua XR was FDA-approved in September 2023 and commercially launched in January 2026 by Aytu BioPharma.
Unlike SSRIs and SNRIs, Exxua XR works through a different mechanism, which contributes to its unique side effect profile. Notably, it's less likely to cause the sexual side effects commonly associated with SSRIs like Sertraline (Zoloft) or Fluoxetine (Prozac).
These are the side effects that were most frequently reported in clinical trials. They tend to be mild to moderate and often improve as your body adjusts to the medication over the first few weeks.
Most of these side effects are dose-related, meaning they may be more noticeable when your dose is increased. Since Exxua XR is started at 18.2 mg and gradually increased up to 72.6 mg, your doctor will titrate slowly to give your body time to adjust.
While uncommon, some side effects require immediate medical attention. Contact your doctor right away — or call 911 in an emergency — if you experience any of the following.
Like all antidepressants, Exxua XR carries an FDA boxed warning about an increased risk of suicidal thoughts and behaviors, particularly in patients under 25 years old. This risk is highest during the first few months of treatment or when doses are changed.
Watch for:
If you or someone close to you notices any of these changes, contact your doctor immediately. If you're in crisis, call the Suicide and Crisis Lifeline at 988.
Exxua XR can affect your heart's electrical rhythm, a condition called QT prolongation. This can lead to dangerous irregular heartbeats. Your doctor should check your baseline QTc interval before starting Exxua XR.
Seek emergency care if you experience:
Taking Exxua XR with other medications that increase serotonin levels can cause a potentially life-threatening condition called serotonin syndrome. This is more of a risk if you're combining Exxua XR with other serotonergic medications.
Symptoms include:
Serotonin syndrome is a medical emergency. Go to the ER immediately if you suspect it.
In patients with bipolar disorder (diagnosed or undiagnosed), Exxua XR can trigger a manic episode. Symptoms include unusually elevated mood, decreased need for sleep, racing thoughts, and risky behavior. Tell your doctor if you have any history of bipolar disorder or mania.
Although rare, allergic reactions can occur. Seek emergency help if you develop a rash, hives, swelling of the face or throat, or difficulty breathing.
Patients 65 and older should start at 18.2 mg and increase slowly (maximum 36.3 mg after 7 days). Older adults may be more sensitive to dizziness and QT prolongation, especially if they're taking other medications.
If you have moderate to severe kidney impairment (CrCl less than 50 mL/min), your maximum dose may be limited to 36.3 mg. Your doctor will adjust your dose based on your kidney function.
Patients with moderate liver impairment (Child-Pugh B) should not exceed 36.3 mg after 7 days. Exxua XR is not recommended for anyone with severe liver impairment (Child-Pugh C).
Exxua XR may harm an unborn baby. Taking it during the third trimester may cause withdrawal symptoms in the newborn. It's not known whether Gepirone passes into breast milk. Discuss the risks and benefits with your doctor if you're pregnant, planning to become pregnant, or breastfeeding.
Here are practical tips for dealing with the most common side effects:
One reason doctors are excited about Exxua XR is its side effect profile. Compared to SSRIs like Sertraline, Escitalopram, or Fluoxetine, Exxua XR is significantly less likely to cause:
These are some of the most common reasons people stop taking their antidepressants, so having an option with a lower risk of these effects is a meaningful advantage for many patients.

Every medication has side effects, and Exxua XR is no exception. The good news is that most of the common side effects — dizziness, nausea, insomnia, and stomach discomfort — are mild and tend to improve with time. The serious side effects are uncommon but important to know about, especially the boxed warning about suicidal thoughts in younger patients and the risk of QT prolongation.
The best thing you can do is stay in close communication with your doctor, especially during the first few weeks and whenever your dose changes. Report any new or unusual symptoms promptly, and never stop taking Exxua XR abruptly without medical guidance.
