Because it's applied directly in the ear and minimally absorbed into the bloodstream, it has a favorable side effect profile compared to systemic antibiotics. But like any medication, it can cause side effects — and some rare ones require immediate medical attention.

Is Ciprofloxacin/Fluocinolone Absorbed Into the Body?

One reassuring feature of Otovel is its minimal systemic absorption. Pharmacokinetic studies in pediatric patients showed negligible detectable levels of either ciprofloxacin or fluocinolone acetonide in plasma after the standard 7-day course. This means systemic (whole-body) side effects are very unlikely. The side effects you need to watch for are primarily local — in and around the treated ear.

Common Side Effects (May Be Normal During Treatment)

The following side effects occurred in 1% or more of patients in clinical trials. Many of these overlap with the symptoms of the infection itself, so it can be difficult to distinguish a side effect from the disease process:

Otorrhea (ear drainage): Fluid draining from the ear is the primary symptom of AOMT itself, so some drainage during treatment is expected. It should decrease over the course of treatment.

Ear pruritus (itching): Mild itching in the ear canal. Usually resolves on its own.

Ear pain or discomfort: Some mild ear pain may occur. Warming the vial in your hand for 1–2 minutes before use reduces this significantly.

Excessive granulation tissue: Some children develop extra tissue near the ear tube. This is relatively common and should be monitored by your ENT.

Auricular swelling: Mild swelling of the outer ear. Usually mild and resolves with treatment completion.

Balance problems or dizziness: Most commonly caused by instilling a cold solution into the ear. Always warm the vial in your hand before use.

Uncommon Side Effects (Less Than 1% of Patients)

Tinnitus (ringing in the ears)

Hypoacusis (reduced hearing)

Tympanic membrane disorder (changes to the eardrum)

Tympanostomy tube obstruction (blockage of the ear tube)

Contralateral otitis media (infection in the other ear)

Serious Side Effects — Seek Medical Attention Immediately

The following are rare but serious. Stop using ciprofloxacin/fluocinolone and seek emergency care if your child shows any signs of:

Severe allergic reaction (anaphylaxis): Hives, rash, swelling of the face/lips/tongue/throat, difficulty breathing, dizziness, fast or irregular heartbeat, loss of consciousness. Fluoroquinolone antibiotics have been associated with life-threatening anaphylactic reactions, some after the first dose.

New or worsening infection: Prolonged use may allow overgrowth of non-susceptible bacteria or fungi. If the ear doesn't improve after 7 days or gets worse, contact your doctor.

What's Normal vs. What Needs Attention

Normal: Continued mild drainage for the first 1–3 days of treatment; mild warmth or itching in the ear; some discomfort when drops are instilled

Call your doctor: Drainage doesn't improve after 7 days; drainage comes back within 6 months; the ear gets significantly worse; new symptoms appear (rash, significant swelling, sudden hearing loss)

Call 911 / go to ER: Signs of anaphylaxis (throat swelling, trouble breathing, loss of consciousness)

Tips to Minimize Side Effects

Always warm the vial in your hand for 1–2 minutes before use — this is the most effective way to prevent dizziness from cold fluid in the ear

Keep the ear facing upward for 1 minute after instillation to allow the medication to penetrate

Complete the full 7-day course even if symptoms improve — stopping early can lead to treatment failure or recurrence

Do not use cotton swabs or other objects in the ear canal — this can push debris into the tube

Most side effects from Otovel are mild, local, and resolve on their own during or after the 7-day treatment course. Ear discomfort, itching, and mild drainage typically improve as the infection clears. If side effects persist after completing the course, or if new symptoms develop, contact your healthcare provider.
