The Most Important Interaction: Drugs That Lower the Seizure Threshold
The most clinically significant interaction with amifampridine involves medications that reduce the brain's resistance to seizures (lower the seizure threshold). Drug classes that lower the seizure threshold and interact with amifampridine include:

Cholinergic Drugs: Increased Risk of Cholinergic Effects
Amifampridine increases acetylcholine (ACh) release at neuromuscular junctions. When combined with drugs that also increase cholinergic activity, the effect may be additive and potentially cause cholinergic toxicity.
Pyridostigmine (Mestinon): Often used together with amifampridine for LEMS — the combination can increase ACh availability at the neuromuscular junction from both sides (presynaptic and postsynaptic).
