What Is Voquezna and Why Was It Prescribed?

Vonoprazan is a potassium-competitive acid blocker (PCAB) — the first of its kind approved in the U.S. It's prescribed for: healing and maintenance of erosive esophagitis, heartburn relief in non-erosive GERD, and eradication of H. pylori (as Voquezna Triple Pak or Dual Pak). Alternatives for GERD and Erosive Esophagitis

Proton pump inhibitors (PPIs) are the most established alternatives to Voquezna for GERD and erosive esophagitis. They've been in use for decades, are widely available as generics, and cost a fraction of Voquezna's price.

Omeprazole (Prilosec)

Omeprazole is the most widely prescribed PPI in the U.S. It's available OTC and as a generic prescription. Typical dose for erosive esophagitis is 20–40 mg once daily for 4–8 weeks. Generic omeprazole costs roughly $5–$15 per month, making it dramatically more affordable than Voquezna. It's appropriate for mild-to-moderate GERD and most cases of erosive esophagitis, though clinical data suggests vonoprazan may be more effective for the most severe cases.

Esomeprazole (Nexium)

Dexlansoprazole uses a dual-release formula that provides extended acid suppression — which can be particularly helpful for nighttime GERD symptoms. Alternatives for H. Pylori Treatment

• Standard triple therapy (PPI + amoxicillin + clarithromycin): The long-standing first-line H. pylori regimen. Uses lansoprazole or omeprazole instead of vonoprazan. Less expensive but slightly lower eradication rates in clarithromycin-resistant strains.

• Bismuth quadruple therapy (Pylera or PPI + bismuth + metronidazole + tetracycline): Recommended when clarithromycin resistance is suspected. Pylera is a combination capsule; still requires a PPI.

• Concomitant therapy (PPI + clarithromycin + amoxicillin + metronidazole): Four drugs given simultaneously for 10–14 days. Used in areas with high resistance.

How Do These Alternatives Compare to Voquezna?

Voquezna has two key advantages over PPIs: it works without requiring food to activate it, and it provides faster and more durable acid suppression. For mild-to-moderate GERD, the difference may be minimal. For severe erosive esophagitis (Grade C/D), the gap is more significant — Voquezna reduced treatment failure by 11–21% compared to PPIs in one major 2024 meta-analysis.
