Solosec (secnidazole) is a convenient, single-dose antibiotic for bacterial vaginosis (BV) and trichomoniasis. Here's what to consider.

Why Solosec Availability Is a Challenge

As of 2026, Solosec is the only FDA-approved single-dose oral treatment for BV and trichomoniasis made by a single manufacturer (Lupin Pharmaceuticals). Retail prices range from $290 to $550 without insurance, and many insurance plans require patients to try cheaper alternatives first — a process called step therapy. For all these reasons, patients often need alternatives.

Alternative 1: Metronidazole (Flagyl) — The First-Line Choice

Metronidazole is the CDC's first-line treatment for both bacterial vaginosis and trichomoniasis. It's in the same drug class as Solosec (nitroimidazoles), is widely available in generic form at virtually every pharmacy, and costs as little as $4–$15 for a full course.

For BV: 500 mg orally, twice daily for 7 days — OR — vaginal gel (MetroGel) 0.75% applied once daily for 5 days

For trichomoniasis: 500 mg orally, twice daily for 7 days — OR — a single 2 g dose

Cost: $4–$15 for a 7-day course of generic tablets

Downside: Requires 7 days of twice-daily dosing instead of a single dose; also requires avoiding alcohol for the duration of treatment plus 48 hours after

Metronidazole is highly effective and has decades of clinical evidence behind it. The main trade-off is adherence — completing all 7 days of treatment is important to prevent the infection from returning or becoming resistant.

Alternative 2: Tinidazole (Tindamax) — Fewer Days, Fewer Side Effects

Tinidazole is another nitroimidazole antibiotic, closely related to both metronidazole and secnidazole. Generic tinidazole is available and costs $15–$50 depending on the course.

For BV: 1 g daily for 5 days, OR 2 g daily for 2 days

For trichomoniasis: 2 g as a single oral dose (closest to Solosec's convenience)

Note: Some studies show tinidazole has higher cure rates than metronidazole for trichomoniasis and is preferred for resistant cases

Alternative 3: Clindamycin — For BV Only (Not Trichomoniasis)

Clindamycin is a lincosamide antibiotic — a completely different drug class from the nitroimidazoles. It is effective for BV but does NOT treat trichomoniasis. It is the CDC's preferred alternative for patients who are allergic to or cannot tolerate metronidazole or tinidazole.

For BV (oral): 300 mg orally, twice daily for 7 days

For BV (vaginal): Clindamycin 2% vaginal cream once daily at bedtime for 7 days (note: oil-based, can weaken latex condoms)

Cost: Oral generic clindamycin: $10–$30 for a 7-day course

Alternative 4: Metronidazole Vaginal Gel (MetroGel)

For patients who want to minimize systemic side effects, metronidazole vaginal gel is applied directly and absorbed locally with less GI impact than oral tablets. It is FDA-approved for BV (not trichomoniasis).

Dose: 0.75% gel, 5 g applied intravaginally once daily for 5 days
