For people with kidney concerns or who are at higher risk for bone loss, Descovy may be the better oral option.

Important limitation: Descovy is NOT approved for individuals at risk from receptive vaginal sex. It's approved for cisgender men who have sex with men and for transgender individuals. Also, there is no FDA-approved US generic as of 2026, making it significantly more expensive — brand-name Descovy runs well over $2,000 per month at list price, though insurance and patient assistance programs can reduce this substantially.

Best for: Cisgender men and transgender individuals at risk from anal sex who have kidney or bone health concerns and have adequate insurance coverage.

Alternative 3: Yeztugo (Lenacapavir) — Every 6 Months

Yeztugo (lenacapavir) is the newest injectable PrEP option, FDA approved in June 2025. It is administered just twice a year — every six months — making it the least frequent dosing option currently available. Clinical trials showed approximately 99% efficacy, with some studies reporting zero HIV infections among participants.

Like Apretude, Yeztugo is administered by a healthcare provider. Because it's newer, provider availability and insurance coverage are still expanding — you may encounter similar access challenges as with Apretude. Long-term real-world data is also still emerging since it's been available less than a year.
