Valganciclovir (Valcyte) is an antiviral used to prevent and treat CMV in transplant patients and people with AIDS. Here's everything you need to know in plain language.

Valganciclovir is an oral antiviral medication best known by its brand name, Valcyte. It is one of the most important drugs used after organ transplantation and in the treatment of a specific eye infection in people with AIDS. Here is everything patients and caregivers need to know about what valganciclovir is, what it does, and how it is used.

What Is Valganciclovir?

Valganciclovir is an antiviral medication in the class of purine nucleoside analogues. It is a prodrug — meaning it is inactive when you take it and gets converted into the active drug (ganciclovir) inside your body. After you swallow a valganciclovir tablet or solution, your intestines and liver convert it to ganciclovir, which then fights the cytomegalovirus (CMV) infection.

Cytomegalovirus is a member of the herpesvirus family. It infects most people at some point in their lives and usually causes no symptoms in healthy individuals. However, in people with weakened immune systems — including organ transplant recipients taking immunosuppressants and people with AIDS — CMV can cause serious, life-threatening infections affecting the eyes, lungs, digestive system, and brain.

What Is Valganciclovir Used For?

Valganciclovir has the following FDA-approved uses:

  1. Treatment of CMV retinitis in adults with AIDS. CMV retinitis is a serious eye infection caused by CMV that can lead to blindness if untreated. Valganciclovir slows CMV's growth to prevent further damage to the retina.
  2. Prevention of CMV disease in adults after kidney, heart, or kidney-pancreas transplant. High-risk transplants (donor CMV-positive / recipient CMV-negative, called D+/R-) carry a greater than 50% risk of developing CMV disease without prophylaxis.
  3. Prevention of CMV disease in children (aged 4 months to 16 years) after kidney transplant, and in children (aged 1 month to 16 years) after heart transplant.

Valganciclovir is also used off-label for CMV prophylaxis in liver transplant recipients, stem cell transplant recipients, and other immunocompromised patients — uses not on the FDA label but supported by clinical evidence.

What Forms Does Valganciclovir Come In?

Valganciclovir is available in two forms:

  • 450 mg tablets: Pink, film-coated oval tablets marked "VGC" on one side and "450" on the other. For adults and older children who can swallow tablets. Store at room temperature (68–77°F).
  • 50 mg/mL oral solution: A powder reconstituted by the pharmacist into a tutti-frutti flavored liquid. Preferred for young children. Must be refrigerated after preparation and used within 49 days.

Typical Valganciclovir Doses

Dosing varies by indication and is always adjusted based on kidney function (creatinine clearance):

  • CMV retinitis induction (active treatment): 900 mg (two 450 mg tablets) twice daily with food for 21 days
  • CMV retinitis maintenance: 900 mg once daily with food
  • Transplant CMV prophylaxis: 900 mg once daily with food for 100–200 days post-transplant (duration varies by transplant type and risk)
  • Pediatric dosing: Calculated using body surface area (BSA) and kidney function (Schwartz formula); maximum dose 900 mg

Always take valganciclovir with food. Food significantly improves absorption (bioavailability is approximately 60% with food). Do not crush, chew, or break tablets.

Who Should NOT Take Valganciclovir

Valganciclovir is contraindicated in:

  • People with a history of serious allergic reaction (anaphylaxis) to valganciclovir, ganciclovir, or any ingredient in the formulation
  • Pregnant women (due to high risk of birth defects)
  • Breastfeeding women (potential harm to nursing infant)

Key Things to Remember When Taking Valganciclovir

  • Always take with food to improve absorption and reduce stomach side effects
  • Never miss doses — consistency is critical to CMV prevention and treatment
  • Get regular blood counts (CBC) and kidney function tests as ordered by your doctor
  • Use appropriate contraception during therapy (women: 30 days post-treatment; men: 90 days post-treatment)
  • Do not substitute brand Valcyte for generic valganciclovir (or ganciclovir capsules for valganciclovir tablets) on a one-to-one basis — dosing is different

Frequently Asked Questions

Valganciclovir is FDA-approved to treat CMV retinitis (a serious eye infection) in adults with AIDS, and to prevent CMV disease in adults and children after kidney, heart, or kidney-pancreas transplants. It is also used off-label for CMV prophylaxis in other transplant types and immunocompromised patients.

Yes. Valcyte is the brand name for valganciclovir hydrochloride. Generic valganciclovir tablets (450 mg) and oral solution (50 mg/mL) are therapeutically equivalent to Valcyte and are available at a fraction of the retail cost with discount coupons.

Valganciclovir prophylaxis after a solid organ transplant typically continues for 100–200 days, depending on the transplant type and the patient's CMV risk (D+/R- patients are highest risk and often receive the longest prophylaxis). Your transplant physician will determine the appropriate duration for your situation.

No. Valganciclovir does not cure CMV infection. It works by slowing the replication of the virus, preventing it from spreading or causing further damage. CMV remains in the body after treatment, and the virus can reactivate if immunosuppression increases or therapy is stopped prematurely.

There is no documented grapefruit interaction with valganciclovir. However, you should always take valganciclovir with food (any food) to improve absorption and reduce gastrointestinal side effects. Ask your pharmacist or doctor if you have questions about specific foods.
