Twyneo is the first FDA-approved fixed-dose combination of tretinoin and benzoyl peroxide for acne. Here's everything you need to know about what it is and how to use it.

Twyneo is a prescription topical cream used to treat acne vulgaris. It's the first and only FDA-approved medication that combines tretinoin and benzoyl peroxide — two of the most proven acne-fighting ingredients — into a single formulation. Here's everything you need to know about what Twyneo is, what it treats, how to use it correctly, and what to expect.

What Is Twyneo?

Twyneo (pronounced "twin-ee-oh") is a topical cream containing two active ingredients:

Tretinoin 0.1%: A retinoid derived from vitamin A that helps unclog pores and speed up skin cell turnover.

Benzoyl peroxide 3%: An antibacterial and keratolytic agent that kills acne-causing bacteria and removes dead skin cells.

Twyneo was developed by Sol-Gel Technologies and is marketed in the U.S. by Mayne Pharma. The FDA approved it on July 27, 2021, making it a relatively recent addition to the acne treatment landscape. It comes as a yellow cream in a 30g pump bottle and is applied once daily.

What Is Twyneo Used For?

Twyneo has one FDA-approved indication: the topical treatment of acne vulgaris in adults and pediatric patients 9 years of age and older. It treats both types of acne lesions:

Inflammatory lesions: Papules (small red bumps), pustules (pimples with pus), and nodules.

Non-inflammatory lesions: Comedones (blackheads and whiteheads).

Twyneo is typically indicated for mild-to-moderate acne and is most commonly prescribed for facial acne, though it can be applied to the back, shoulders, and chest as well.

What Makes Twyneo Different From Other Acne Treatments?

Tretinoin and benzoyl peroxide have each been used for acne for decades, but they could never be mixed together in a single product — because benzoyl peroxide chemically degrades tretinoin on contact, reducing its effectiveness. Twyneo solved this with a patented microencapsulation technology: silica (silicon dioxide) core-shell structures that separately encapsulate the two ingredients, keeping them stable until they reach the skin and are released over time.

This means patients get the full benefit of both ingredients in a single once-daily application, with the added advantage of controlled release that may improve tolerability compared to using them separately.

How Well Does Twyneo Work?

Twyneo's FDA approval was based on two phase 3 double-blind, randomized, vehicle-controlled trials involving 858 subjects 9 years and older with moderate-to-severe acne. Results at 12 weeks:

In Trial 1: 38.5% of Twyneo patients achieved IGA success (clear or almost clear skin) vs. 11.5% in the vehicle (placebo) group.

Significant reductions in both inflammatory and non-inflammatory lesion counts were observed.

Some patients saw improvement as early as 2 weeks into treatment.

How Do You Use Twyneo? Dosage and Application

Twyneo's manufacturer recommends a simple 4-step application routine (C-A-M-P):

Cleanse: Gently wash the affected skin, rinse with warm water, and pat dry.

Apply: Press the pump to dispense a small amount. Apply a thin layer to acne-affected areas once daily.

Moisturize: Apply a gentle moisturizer to help manage any dryness or irritation.

Protect: Apply SPF 30+ sunscreen every morning. Twyneo increases skin sensitivity to sunlight.

Wash your hands immediately after applying Twyneo. Avoid the eyes, mouth, and nostrils. Do not apply to broken, sunburned, or eczema-affected skin.

How Should Twyneo Be Stored?

After picking up Twyneo from the pharmacy, store it at room temperature between 68°F and 77°F (20°C–25°C). Do not freeze. Discard Twyneo 60 days after opening or 12 weeks from the date your pharmacy dispensed it — whichever comes first. Pharmacies typically keep Twyneo refrigerated before dispensing.

Who Should NOT Use Twyneo?

Twyneo is contraindicated in patients with a history of serious hypersensitivity reaction to benzoyl peroxide or any component of Twyneo. It should not be used in children under age 9. Use with caution during pregnancy — discuss with your doctor. Safety in breastfeeding has not been established.

Frequently Asked Questions

Twyneo is FDA-approved for the topical treatment of acne vulgaris (common acne) in adults and children 9 years of age and older. It treats both inflammatory lesions (papules, pustules) and non-inflammatory lesions (blackheads and whiteheads). It is applied to affected areas of the face, back, shoulders, and chest.

No. Twyneo contains tretinoin (0.1%) as one of its active ingredients, but it also contains benzoyl peroxide (3%). The combination is the key innovation — tretinoin alone does not have antibacterial properties. Twyneo uses microencapsulation technology to keep the two ingredients stable together, something that was not previously possible.

Some patients see improvement as early as 2 weeks after starting Twyneo. However, full results typically require 12 weeks of consistent once-daily use. A temporary worsening of acne (purging) is common in the first 2–4 weeks as the medication clears out existing blockages in pores. Don't stop using it unless you experience a serious adverse reaction.

Yes. Twyneo is FDA-approved for use in patients 9 years of age and older, including teenagers. Clinical trials included pediatric subjects 9 years and older and demonstrated safety and efficacy. It is not approved for children under age 9.

There is insufficient data on Twyneo's safety during pregnancy. Animal studies with topical tretinoin at high doses showed fetal abnormalities, though systemic absorption from topical application is very low. Tell your doctor if you are pregnant or planning to become pregnant. Your provider will weigh the risks and benefits before recommending whether to continue or discontinue.
