Pravastatin (Pravachol) is a statin used to lower cholesterol and prevent heart disease. Get a plain-language overview of what it does, how to take it, and what to watch for.

Pravastatin is a prescription medication used to lower cholesterol and reduce the risk of heart disease, heart attack, and stroke. It belongs to a class of drugs called statins (HMG-CoA reductase inhibitors) — the most widely prescribed group of cholesterol-lowering medications in the world. Here's everything you need to know about Pravastatin in plain language.

Brand Name and Generic

Pravastatin was originally sold under the brand name Pravachol (Bristol-Myers Squibb). The brand-name version is no longer sold in the United States — only generic Pravastatin is currently available. Generic Pravastatin has been available since April 2006 and is manufactured by more than a dozen companies.

What Is Pravastatin Used For?

Pravastatin is FDA-approved for several uses:

High cholesterol (hypercholesterolemia): Lowers LDL ("bad") cholesterol and total cholesterol while raising HDL ("good") cholesterol.

High triglycerides and mixed dyslipidemia: Reduces elevated blood fats that increase cardiovascular risk.

Cardiovascular event prevention: Reduces the risk of heart attack, stroke, and need for heart procedures (revascularization) in adults with or at high risk for coronary artery disease.

Familial hypercholesterolemia in children: FDA-approved for children aged 8 and older with heterozygous familial hypercholesterolemia who don't respond to diet changes alone.

Pravastatin should always be used together with a heart-healthy diet and regular exercise. It is not a substitute for lifestyle changes — it works best as an adjunct to them.

How Effective Is Pravastatin at Lowering Cholesterol?

Pravastatin is classified as a low-to-moderate intensity statin:

10–20 mg dose: Reduces LDL cholesterol by less than 30% (low intensity)

40–80 mg dose: Reduces LDL cholesterol by 30–50% (moderate intensity)

While Pravastatin is less potent than Atorvastatin or Rosuvastatin, it has a highly favorable safety profile and is appropriate for many patients — particularly those who need moderate LDL reduction or who have drug interactions that make more potent statins less suitable. It has undergone more than 112,000 patient-years of double-blind clinical trials.

Pravastatin Dosage: How Much Do You Take?

Pravastatin comes in four tablet strengths: 10 mg, 20 mg, 40 mg, and 80 mg. It is taken by mouth once daily.

Adults: Typical starting dose is 40 mg once daily. Doses can be adjusted up to 80 mg or down to 10 mg based on response and tolerability. If you have severe kidney problems or are taking cyclosporine, start with 10 mg.

Children 8–13 years: 20 mg once daily (maximum 20 mg/day in this age group)

Adolescents 14–18 years: 40 mg once daily

How to Take Pravastatin

Timing: Pravastatin can be taken any time of day, but evening administration is slightly preferred because the body produces more cholesterol at night. Unlike some other statins, this is a preference, not a requirement.

With or without food: Can be taken with or without food. Absorption is slightly decreased with food but this does not reduce the clinical cholesterol-lowering effect.

Bile acid sequestrants: If you also take cholestyramine (Questran) or colestipol (Colestid), take those at least 4 hours before or 1 hour after Pravastatin to avoid absorption issues.

Missed dose: If you miss a dose, take it as soon as you remember. If it's almost time for your next dose, skip the missed dose. Never double up.

Important Safety Information

Not for active liver disease: Do not take Pravastatin if you have active liver disease or persistently elevated liver enzymes of unknown cause.

Pregnancy and breastfeeding: Do not take Pravastatin if you are pregnant. Stop taking it and tell your doctor if you become pregnant. Do not breastfeed while taking Pravastatin.

Muscle symptoms: Contact your doctor if you experience unexplained muscle pain, weakness, or dark-colored urine.

In children aged 8 and older, it is approved for hereditary high cholesterol (heterozygous familial hypercholesterolemia).

The typical starting dose for adults is 40 mg once daily. The dose can be adjusted from 10 mg to 80 mg based on your cholesterol targets, how well you tolerate the medication, and any drug interactions. Dose adjustments are made at 4-week intervals based on your lipid panel results.

Yes. Pravachol was the brand name for Pravastatin, originally manufactured by Bristol-Myers Squibb. The brand-name version is no longer sold in the United States. Only generic Pravastatin (pravastatin sodium) is currently available, manufactured by multiple generic drug companies.

Most patients begin to see meaningful LDL cholesterol reductions within 4 weeks of starting Pravastatin. Your doctor will typically order a follow-up lipid panel 6-8 weeks after starting therapy to assess your response and adjust the dose if needed. Maximum effect is generally reached within 4-8 weeks.
