If your doctor has just prescribed Plavix (or its generic, clopidogrel), you probably have questions. What does it do? How do you take it? How long will you need it? This guide answers the most important questions so you can start your treatment with confidence.
What Is Plavix (Clopidogrel)?
Plavix is the brand name for clopidogrel bisulfate, an antiplatelet medication manufactured by Sanofi-Aventis. It was first approved by the FDA in 1997 and has become one of the most widely prescribed medications in the world. Generic clopidogrel has been available since 2012 after Plavix's patent expired.
Clopidogrel belongs to a class of drugs called P2Y12 platelet inhibitors (or thienopyridines). It prevents blood clots by stopping platelets — small blood cells — from sticking together to form dangerous clots inside arteries.

What Is Clopidogrel Used For?
Clopidogrel has several FDA-approved uses:
- After a heart attack or unstable angina: Clopidogrel (usually combined with aspirin) reduces the risk of additional heart attacks, stroke, and cardiovascular death in patients with acute coronary syndrome (ACS).
- After a coronary stent (PCI): Clopidogrel plus aspirin (dual antiplatelet therapy, or DAPT) is essential for preventing stent thrombosis — a life-threatening clot inside the stent. Duration is typically 6-12 months.
- After a stroke or TIA: Clopidogrel reduces the risk of a future ischemic stroke in patients who have had a prior stroke or mini-stroke (TIA). It is used as secondary prevention.
- Peripheral arterial disease (PAD): Clopidogrel reduces major adverse cardiovascular events in patients with symptomatic PAD. The 2024 ACC/AHA PAD guidelines endorse it as a first-line antiplatelet agent.
Clopidogrel Dosage: How Do You Take It?
Clopidogrel comes in two tablet strengths:
- 75 mg tablet: The standard maintenance dose. Taken once daily, with or without food.
- 300 mg tablet: Used as a loading dose in acute situations — such as before or during a PCI procedure — to rapidly achieve platelet inhibition. Sometimes 600 mg (two 300 mg tablets) is used for faster loading.
For most patients on long-term therapy: take your 75 mg tablet at the same time each day. You can take it with or without food. If you are also taking aspirin, take it as directed by your doctor — do not stop aspirin without checking first.
What Happens If You Miss a Dose?
If you miss a dose, take it as soon as you remember. If it is almost time for your next scheduled dose, skip the missed dose and continue with your regular schedule. Do not double up doses. If you miss multiple doses or are running out of medication, contact your prescriber immediately.
How Long Will You Need to Take Clopidogrel?
The duration depends on why you are taking it:
- After a drug-eluting stent: typically 6-12 months of dual antiplatelet therapy (DAPT), sometimes longer for high-risk patients
- After a bare metal stent: at least 1 month of DAPT
- After a stroke or TIA: often long-term (years), sometimes indefinitely
- For PAD: often indefinitely as long as it is well tolerated
Never stop clopidogrel without your doctor's approval — even if you feel fine. The risk of stopping prematurely can be severe.

- Any history of bleeding disorders, stomach ulcers, or intracranial hemorrhage
- Any planned surgeries, dental procedures, or invasive tests

Frequently Asked Questions
Plavix is the brand name; clopidogrel is the generic name. They contain the same active ingredient at the same dosage. The FDA requires generics to be bioequivalent to the brand, meaning they work the same way. Generic clopidogrel costs 80-90% less than brand-name Plavix and is the version dispensed at most pharmacies.
Clopidogrel is FDA-approved to prevent blood clots in patients with acute coronary syndrome (heart attack or unstable angina), after coronary stent placement (PCI), after an ischemic stroke or TIA, and in patients with peripheral arterial disease. It is also used off-label to prevent clots in patients with atrial fibrillation in certain situations.
Yes. Clopidogrel can be taken with or without food. Taking it with food may reduce stomach upset for some patients. The presence of food does not significantly affect how well the medication works.
Clopidogrel begins inhibiting platelets on the first day of use, but maximum effect takes 3-7 days at the standard 75 mg daily dose. A loading dose (300-600 mg) achieves faster platelet inhibition — often within 2 hours — which is why loading doses are used in urgent situations like stent procedures.
Stopping clopidogrel suddenly — especially within the recommended treatment window after a stent — can lead to stent thrombosis, a life-threatening clot inside the stent. Even for other indications, abrupt discontinuation increases clotting risk. Always consult your doctor before stopping, even temporarily.
