What Is Milnacipran?

Milnacipran is the generic name for the brand-name drug Savella. It belongs to a class of drugs called serotonin-norepinephrine reuptake inhibitors (SNRIs) — the same class as Cymbalta (duloxetine) and Effexor (venlafaxine), which are used primarily as antidepressants.

The FDA approved Milnacipran in January 2009 specifically for the management of fibromyalgia in adults. This makes it one of only three FDA-approved drugs for fibromyalgia in the United States, alongside duloxetine (Cymbalta) and pregabalin (Lyrica). In Europe, Milnacipran is also approved for depression — but this use is not FDA-approved in the US.

Milnacipran is not a controlled substance. It does not have abuse potential and can be prescribed by any licensed healthcare provider, including via telehealth.

What Is Fibromyalgia?

Fibromyalgia is a chronic condition characterized by widespread musculoskeletal pain, fatigue, sleep problems, and often cognitive difficulties (sometimes called "fibro fog"). It affects an estimated 4 million Americans, predominantly women. Fibromyalgia is not detectable on standard blood tests or imaging — it's diagnosed based on symptoms and clinical criteria.

How Is Milnacipran Taken? Dosage and Titration Schedule

Milnacipran has a specific titration schedule designed to minimize side effects like nausea:

Day 1: 12.5 mg once

Days 2-3: 12.5 mg twice daily

Days 4-7: 25 mg twice daily

Day 8 and beyond (maintenance): 50 mg twice daily (100 mg/day total)

Maximum dose: 200 mg/day (doses above 200 mg have not been studied)

Milnacipran comes as film-coated tablets in strengths of 12.5 mg, 25 mg, 50 mg, and 100 mg. It can be taken with or without food, but taking it with food significantly reduces nausea.

How Well Does Milnacipran Work?

In randomized controlled trials, 37% of patients receiving Milnacipran reported at least a 50% reduction in pain intensity, compared to 14% with placebo. Milnacipran was also shown to improve fatigue, physical function, global impression of disease, and quality of life compared to placebo.

However, like all fibromyalgia medications, Milnacipran works well for some patients and not others. A 2015 systematic review showed moderate relief for a minority of people with fibromyalgia — and one-fourth of surveyed patients in a patient organization study rated it as beneficial for pain and fatigue. It may take several weeks to months before the full effect is felt.

Who Should NOT Take Milnacipran?

Patients taking or recently stopped MAOIs (at least 14-day washout required before starting Milnacipran; 5-day washout after stopping Milnacipran before starting an MAOI)

Patients with significant alcohol use disorder or chronic liver disease

Patients under 18 (not studied; not recommended)

Pregnant women (animal studies showed embryofetal risk; no adequate human data)

Patients with known hypersensitivity to milnacipran
