Common Side Effects of Urocit-K XR

The most common side effects are gastrointestinal in nature. These occur because potassium citrate can irritate the lining of the digestive tract as the tablet passes through. Common GI side effects include:

Abdominal discomfort or stomach pain

Nausea

Vomiting

Diarrhea

Loose stools

You may also notice intact wax matrix "shells" in your stool. This is normal and expected — it's just the outer casing of the extended-release tablet after the active ingredient has been absorbed. This does

not mean the medication isn't working.

How to Minimize GI Side Effects

Gastrointestinal side effects are often dose-related and can be managed with a few simple strategies:

Always take Urocit-K XR with food or within 30 minutes after a meal or snack. Taking it on an empty stomach significantly increases the risk of nausea and abdominal pain.

Ask your doctor about splitting the dose. Taking smaller doses more frequently (e.g., three times daily instead of twice daily) can reduce the GI impact at each dose.

Stay hydrated. Drinking at least 2 liters of water per day is recommended regardless — it also helps your GI system handle the tablets better.

Serious Side Effects of Urocit-K XR

While rare, the following side effects are potentially serious and require immediate medical attention:

1. Hyperkalemia (High Potassium)

This is the most dangerous potential side effect of Urocit-K XR. Because the medication delivers potassium into your bloodstream, it can elevate blood potassium levels — especially in patients with impaired kidney function who can't excrete potassium efficiently. Hyperkalemia can cause abnormal heart rhythms and, in severe cases, cardiac arrest. Importantly, hyperkalemia can develop rapidly and with

no symptoms — which is why regular blood monitoring is essential.

Symptoms of hyperkalemia can include: muscle weakness, fatigue, unusual tiredness, heart palpitations, numbness or tingling, and difficulty breathing. Seek emergency care immediately if you experience these symptoms while on Urocit-K XR.

2. GI Bleeding and Ulceration

Potassium salts can cause bleeding ulcers in the esophagus, stomach, or intestines — particularly in patients with a history of peptic ulcer disease or those taking anticholinergic medications (which slow GI transit). Signs include: blood in stool or vomit, black/tarry stools, severe abdominal pain, or vomiting blood. Call your doctor or seek emergency care if these occur.

3. GI Obstruction or Narrowing

The wax-matrix tablets can cause obstruction in patients who have delayed gastric emptying, esophageal compression, or intestinal strictures. If you have any of these conditions, do not take Urocit-K XR without first discussing it with your doctor. Warning signs include difficulty swallowing, feeling of tablets getting stuck, or severe abdominal pain after taking the medication.

Monitoring While on Urocit-K XR

Your prescriber should monitor you every four months with:

Serum electrolytes (sodium, potassium, chloride, bicarbonate)

Serum creatinine (to assess kidney function)

Complete blood count (CBC)

24-hour urinary citrate and urinary pH measurements

Periodic ECGs, especially for patients with cardiac disease or renal disease
