Permethrin Side Effects: What to Expect and When to Call Your Doctor

Permethrin is very safe, but burning, itching, and stinging after application are common. Learn which side effects are normal and which need medical attention.

Permethrin is one of the safest topical antiparasitic medications available. Less than 2% is absorbed through intact skin, making systemic side effects extremely rare. But "safe" doesn't mean side-effect-free — and knowing what to expect after applying permethrin can help you avoid unnecessary anxiety (or missing something genuinely concerning).

Most Common Side Effects of Permethrin

These reactions are expected and typically mild and temporary. They are most common immediately after application and in the first few days of treatment:

Burning and stinging — Reported in approximately 10% of patients in clinical trials. Usually mild and limited to areas with broken skin or active rash.

Itching (pruritus) — Reported in about 7% of patients after application. Importantly, itching after permethrin treatment does NOT mean the treatment failed. Post-treatment itch is the body's reaction to dead mites and their byproducts and can persist for up to 4 weeks.

Redness (erythema) — Mild skin redness at the application site. Reported in 1-2% of patients.

Numbness or tingling — A mild tingling sensation on the skin is a known effect of pyrethroid medications; usually temporary.

Mild rash — Temporary rash or skin irritation at the site of application. Distinct from the original scabies rash.

Why Am I Still Itching After Permethrin Treatment?

This is the most common concern patients have after using permethrin — and the answer is reassuring. Itching after permethrin treatment is almost always normal and expected, not a sign of treatment failure.

When permethrin kills scabies mites, their remains are left in the skin. Your immune system continues reacting to these dead mites for several weeks — hence the itching. In clinical trials, approximately 75% of patients who still had itching at 2 weeks were itch-free by 4 weeks. You only need to worry if:

You can see live mites burrowing under the skin 14 days after treatment

New burrows appear in new locations after treatment

Symptoms worsen significantly rather than gradually improving over time

Rare Side Effects Reported After Permethrin Use

The following side effects have been reported post-marketing but are rare and not expected with normal, directed use:

Headache, fever, dizziness

Abdominal pain, diarrhea, nausea, or vomiting

Rare occurrences of seizure (none were confirmed as causally related to permethrin in clinical reporting)

These reactions are extremely uncommon with topical application because less than 2% of permethrin is absorbed systemically.

Serious Side Effects: Allergic Reactions

The most serious potential side effect of permethrin is a severe allergic reaction (hypersensitivity). This is rare but requires immediate attention. Signs of a serious allergic reaction include:

Severe hives, rash, or swelling (especially of the face, lips, tongue, or throat)

Difficulty breathing or swallowing

Dizziness or fainting

Rapid heartbeat

If you experience any of these, stop using permethrin immediately and call 911 or go to the nearest emergency room. Permethrin should not be used by anyone with a known allergy to pyrethrins, pyrethroids, or chrysanthemums.

Is Permethrin Safe During Pregnancy?

Yes. Permethrin 5% cream is the CDC-preferred treatment for scabies and lice during pregnancy. It carries a former FDA Pregnancy Category B designation — animal studies showed no harm to fetuses at standard doses, and it is not expected to pose risk to humans. The systemic absorption is less than 2%, limiting fetal exposure.

Is Permethrin Safe for Children?

Permethrin 5% cream is FDA-approved and safe for children 2 months of age and older. The 1% lotion (for head lice) can be used in children 2 years and older without special precautions. Do not use either product in infants younger than 2 months without physician guidance.

When Should I Call My Doctor After Using Permethrin?

Call your doctor if:

Live mites or lice are still visible 14 days after completing treatment

Skin irritation is severe, worsening, or not improving after 1 week

You develop signs of a skin infection (increasing warmth, swelling, pus, or fever)

You experience any signs of an allergic reaction
