1. Azathioprine and 6-Mercaptopurine (Thiopurines)

When you take mesalamine with these drugs, levels of the thiopurine's toxic metabolites can build up in your system — increasing the risk of serious blood problems including bone marrow suppression, low white blood cell count (neutropenia), low platelet count, and anemia.

Both azathioprine and 6-MP are commonly prescribed for IBD. If you take either of these with Pentasa XR, your doctor should monitor your complete blood cell count (CBC) and platelets regularly.

2. Varicella (Chickenpox) Vaccine — Live

Because mesalamine is a salicylate, using it around the time of a live varicella vaccination (chickenpox vaccine) can increase the risk of Reye's syndrome — a rare but serious condition affecting the brain and liver. You should avoid mesalamine for at least 6 weeks after receiving the varicella vaccine. Alert your doctor or pharmacist if you receive this vaccine while taking Pentasa XR.

Moderate Drug Interactions (Use with Caution, Monitor Closely)

3. NSAIDs (Ibuprofen, Naproxen, and Other Anti-Inflammatories)

Both NSAIDs and mesalamine can have nephrotoxic (kidney-damaging) effects. Taking them together creates an additive risk of kidney damage. If you regularly take ibuprofen (Advil, Motrin), naproxen (Aleve), aspirin at anti-inflammatory doses, or other NSAIDs for pain or arthritis, discuss this with your doctor. Your kidney function should be monitored more closely if both are needed.

4. Proton Pump Inhibitors (PPIs) — Omeprazole, Pantoprazole, Esomeprazole

PPIs work by reducing stomach acid. This increases the pH in your GI tract, and while Pentasa XR uses an ethylcellulose coating that doesn't depend purely on pH, higher GI tract pH can still affect drug release timing and potentially reduce therapeutic effectiveness. The FDA labeling specifically notes this interaction as a concern to discuss with your provider. If possible, PPIs should be avoided or used cautiously with Pentasa XR.

5. Nephrotoxic Agents (Vancomycin, Aminoglycosides, Cyclosporine)

Any drug that independently damages the kidneys carries an increased risk when combined with mesalamine. This includes certain antibiotics (vancomycin, gentamicin), antifungals (amphotericin B), and immunosuppressants (cyclosporine, tacrolimus). If you require any of these medications, alert your treating team that you are on Pentasa XR and ensure kidney function is monitored closely.

6. Warfarin and Blood Thinners

Mesalamine may increase the anticoagulant effect of warfarin. If you take warfarin (Coumadin) for blood clotting conditions, starting or stopping Pentasa XR could affect your INR (a measure of how quickly your blood clots). Tell your anticoagulation provider about this change and have your INR checked more frequently when making adjustments.

7. Mycophenolate (CellCept, Myfortic)

Mesalamine can increase levels of mycophenolate in the blood by competing for the same renal tubular clearance pathway. This may increase mycophenolate's immunosuppressive effect and potential for side effects. This interaction is particularly relevant for organ transplant patients on mycophenolate.

What to Tell Your Doctor and Pharmacist

Before starting Pentasa XR, give your provider a complete list of all medications you take — including:

  • All prescription medications
  • Over-the-counter medications (especially NSAIDs like ibuprofen or aspirin)
  • Vitamins and herbal supplements
  • Recent vaccinations (particularly varicella/chickenpox vaccine)
  • Any history of kidney or liver disease
