Serious skin reactions (Stevens-Johnson Syndrome / Toxic Epidermal Necrolysis): Rare but life-threatening. Symptoms include a new rash, skin blistering or peeling, sores in the mouth or on the lips, fever, and eye irritation. Stop Nuvigil immediately and seek emergency care at the first sign.

DRESS syndrome (Drug Reaction with Eosinophilia and Systemic Symptoms): A serious multi-organ hypersensitivity reaction characterized by fever, widespread rash that peels, swollen lymph nodes, and abnormal blood counts. Can affect the liver and kidneys. Discontinue immediately if suspected.

Anaphylaxis or severe allergic reaction: Symptoms include hives, swelling of the face, lips, tongue, or throat, difficulty swallowing or breathing, and hoarseness. Call 911 or go to the emergency room immediately.

Psychiatric symptoms: Hallucinations, mania, depression, unusual agitation, aggression, and — in rare cases — suicidal thoughts. Tell your doctor immediately if you experience mood or behavior changes you or your family find concerning.

Chest pain, irregular heartbeat, or significantly elevated blood pressure: Seek care immediately. Armodafinil is not recommended for patients with unstable cardiovascular disease.

Who Is at Higher Risk for Side Effects?

Certain patients should use extra caution with armodafinil:

Patients with a history of psychosis, depression, or bipolar disorder — psychiatric symptoms may emerge or worsen

Patients with cardiovascular disease, hypertension, or arrhythmias — higher risk of cardiac side effects

Patients with severe liver impairment — armodafinil clearance is significantly reduced; a lower dose is typically required

Elderly patients — armodafinil elimination is slowed; start at lower doses

Pregnant patients — safety not established; a pregnancy registry exists if you become pregnant while taking armodafinil
