Like all combination oral contraceptives, Minastrin 24 Fe and its generic equivalents (Charlotte 24 Fe, Mibelas 24 Fe, etc.) come with both common, manageable side effects and a smaller set of serious risks that require prompt medical attention. Knowing the difference is important.

The Boxed Warning: Smoking and Cardiovascular Risk

Minastrin 24 Fe carries a boxed warning—the FDA's most serious type of warning—about the combination of cigarette smoking and combination oral contraceptive use. Cigarette smoking increases the risk of serious cardiovascular events including heart attacks and strokes. This risk increases with age and the number of cigarettes smoked. Combination oral contraceptives, including Minastrin 24 Fe, should not be used by women who are over 35 years of age and smoke.

Common Side Effects (≥2% in Clinical Trials)

In clinical trials with 743 women taking norethindrone acetate 1 mg/EE 20 mcg tablets (the same hormones as Minastrin 24 Fe), these were the most commonly reported side effects:

Headache (6.3%) — Most common; often improves after the first 2-3 months

Vaginal candidiasis (6.1%) — Yeast infections; hormonal changes can alter vaginal pH

Nausea (4.6%) — Most common in the first 1-3 months; taking with food or at bedtime helps

Menstrual cramps (4.4%)

Breast tenderness (3.4%) — Usually mild and temporary

Bacterial vaginitis (3.1%)

Acne (2.7%) — Note: some patients experience acne improvement on COCs; results vary

Mood swings (2.2%)

Weight gain (2.0%) — Often minimal; may be due to fluid retention

Additionally, 24-35% of women in clinical trials experienced unscheduled bleeding (breakthrough bleeding or spotting) per cycle, especially in the first few months. Amenorrhea (no period during the hormone-free interval) occurred in 22-36% of women in at least one of the first 6 cycles.

Serious Side Effects: Seek Medical Attention Immediately

While serious side effects are rare, they do occur with all combination oral contraceptives. Call 911 or go to an emergency room immediately if you experience:

Chest pain or tightness — Could indicate a blood clot in the lungs (pulmonary embolism) or a heart attack

Sudden severe headache — Could indicate a stroke

Vision changes, loss of vision — Could indicate stroke or blood clot affecting vision

Leg swelling, pain, or warmth — Could indicate deep vein thrombosis (DVT)

Shortness of breath — Could indicate pulmonary embolism

Severe abdominal pain — Could indicate liver problems (rare benign hepatic adenoma)

Jaundice (yellowing of skin or eyes) — Could indicate liver disease

These warrant a non-urgent call or message to your provider:

Persistent or worsening headaches that are different from your usual pattern

Depression or significant mood changes persisting beyond the first 2-3 months

Two consecutive missed periods (rule out pregnancy)

Persistent breakthrough bleeding after the first 3 months

Significant blood pressure elevation

Who Should Not Take Minastrin 24 Fe?

Minastrin 24 Fe is contraindicated in patients with: history of VTE or arterial thromboembolism, women over 35 who smoke, severe liver disease or liver tumors, uncontrolled hypertension, diabetes with vascular complications, known hypersensitivity to any component, or active pregnancy.
