Positive pregnancy test or missed periods after previous regularity: Pregnancy while using Kyleena is uncommon but possible. If you become pregnant with the device in place, there is a risk of ectopic pregnancy. Seek immediate medical attention.

Sharp or sudden abdominal pain (especially after insertion): This may indicate uterine perforation — the device has gone through the wall of the uterus. Though rare (<0.1%), it requires immediate care.

Side Effects Specific to the First Days After Insertion

In the 24–48 hours after insertion, it's normal to experience:

Cramping similar to menstrual cramps (managed with ibuprofen)

Light spotting or bleeding

Dizziness or lightheadedness immediately after insertion (usually brief)
