Learn about Cefprozil drug interactions including medications, supplements, and foods to watch out for. Know what to tell your doctor before starting.
Before starting any antibiotic, it's important to understand how it might interact with other medications you're taking. Cefprozil (formerly brand name Cefzil) is generally well-tolerated and has fewer drug interactions than many other antibiotics — but there are still some important ones to know about.
This guide covers the major and moderate drug interactions, supplements and OTC medications to watch, food considerations, and what to tell your doctor before starting Cefprozil.
A drug interaction happens when one substance affects how another substance works in your body. Interactions can:
Cefprozil is primarily eliminated by the kidneys and has relatively few metabolic interactions. But "few" doesn't mean "none" — the interactions below are clinically significant.
1. Probenecid
Probenecid is a medication used to treat gout. It works by reducing how much uric acid your kidneys excrete. The problem: it also reduces how quickly your kidneys clear Cefprozil. Taking both together increases Cefprozil blood levels, which raises the risk of side effects like nausea, diarrhea, and (rarely) seizures.
If you take Probenecid, your doctor may need to adjust your Cefprozil dose or monitor you more closely.
2. Aminoglycoside Antibiotics
Aminoglycosides like Gentamicin, Tobramycin, and Amikacin are powerful injectable antibiotics. When combined with cephalosporins like Cefprozil, there's an increased risk of kidney damage (nephrotoxicity). This combination is sometimes used in serious infections under close medical supervision, but your kidney function should be monitored.
3. Loop Diuretics
Loop diuretics like Furosemide (Lasix) and Bumetanide (Bumex) are used to treat fluid retention and high blood pressure. When taken with cephalosporins, they may increase the risk of nephrotoxicity. If you're on a loop diuretic and prescribed Cefprozil, your doctor should be aware.
4. Warfarin (Coumadin, Jantoven)
Cefprozil, like many antibiotics, may enhance the blood-thinning effect of Warfarin. This happens because antibiotics can alter gut bacteria that produce vitamin K, which plays a role in blood clotting. If you take Warfarin, your doctor should monitor your INR (a measure of blood clotting) more frequently while you're on Cefprozil.
5. Hormonal Contraceptives (Birth Control Pills)
There's a theoretical concern that antibiotics like Cefprozil may reduce the effectiveness of hormonal birth control by disrupting gut bacteria involved in estrogen recycling. While the evidence is limited, many healthcare providers recommend using backup contraception (like condoms) while taking any antibiotic and for 7 days after finishing the course.
6. Live Vaccines
Live vaccines like BCG and oral typhoid vaccine may be less effective when given during antibiotic treatment. Cefprozil can kill bacteria used in live bacterial vaccines, reducing their ability to trigger an immune response. If you need a live vaccine, discuss timing with your doctor.
Cefprozil doesn't have many supplement interactions, but keep these in mind:
The good news: Cefprozil has no significant food interactions. You can take it with or without food, with any type of meal.
A few notes:
Before your doctor prescribes Cefprozil, make sure they know about:

Cefprozil is one of the safer antibiotics when it comes to drug interactions. The main ones to watch are Probenecid (increases drug levels), aminoglycosides and loop diuretics (kidney risk), and Warfarin (bleeding risk). There are no significant food interactions, and most supplements are safe to take alongside it.
The key takeaway: always give your doctor and pharmacist a complete list of what you're taking. They can check for interactions and adjust your treatment plan if needed.
