While it is not FDA-approved for VRE infections (only linezolid has that specific approval), daptomycin is widely used off-label for serious VRE infections and is considered a first-line agent by many infectious disease specialists.

  • Used for: VRE bloodstream infections (off-label), MRSA skin infections, endocarditis, bacteremia
  • How given: IV only — no oral form available
  • Cannot treat pneumonia: Lung surfactant inactivates daptomycin. If your infection is pneumonia, daptomycin is not an option.

Alternative 3: Tedizolid (Sivextro) — Same Class, Once Daily Dosing

Tedizolid (Sivextro) is a newer oxazolidinone antibiotic — the same drug class as linezolid. It was FDA-approved in 2014 for acute bacterial skin and skin structure infections (ABSSSIs). Because it shares the same mechanism of action as linezolid, it may be a direct substitution candidate for certain skin infections.

  • Advantage: Once-daily dosing (vs. twice daily for linezolid); available as both IV and oral
  • Limitation: Only FDA-approved for skin infections; not approved for pneumonia or VRE
  • Cost: Brand-only (no generic) and significantly more expensive than generic linezolid with a coupon

Alternative 4: Telavancin (Vibativ) — For Hospital-Acquired Pneumonia

Telavancin is a lipoglycopeptide antibiotic chemically related to vancomycin but designed to be more potent. It is FDA-approved for hospital-acquired and ventilator-associated bacterial pneumonia caused by MRSA, and for complicated skin infections.

  • Used for: Hospital-acquired MRSA pneumonia, complicated skin infections
  • How given: IV only; limited to hospital settings
  • Risk: Risk of kidney toxicity; not widely used outside hospital settings
