Headache (3%): Usually mild and most common in the first week. Taking Alfuzosin XR consistently with the same meal each day can help reduce headache frequency.

Fatigue (3%): Tiredness or weakness, particularly in the first few weeks. Usually improves as your body adjusts.

Nausea, abdominal pain: Occur in 1-2% of patients. Taking the tablet immediately after a full meal (not on an empty stomach) significantly reduces digestive side effects.

Serious Side Effects: When to Seek Medical Attention

Orthostatic hypotension (postural low blood pressure): A significant drop in blood pressure when standing up, which can cause dizziness, lightheadedness, or fainting. Most likely to occur within the first few hours after taking a dose, or if you're also taking antihypertensive medications. If you feel faint, sit or lie down with your legs elevated. Call your doctor if this happens repeatedly.

Angina (chest pain): If you develop new or worsening chest pain while taking Alfuzosin XR, stop taking it and seek medical attention immediately. The FDA label specifically states to discontinue if angina symptoms appear or worsen.

QT prolongation: Alfuzosin can slightly prolong the QT interval on an electrocardiogram. What Makes Alfuzosin XR Different: Ejaculatory Side Effects

One important advantage of Alfuzosin XR compared to other alpha-blockers is its low rate of ejaculatory side effects. Retrograde ejaculation and decreased ejaculatory volume — common with tamsulosin (8-18%) and silodosin (>20%) — occur in only about 1-3% of patients taking alfuzosin. This is one reason many prescribers choose alfuzosin specifically for sexually active men.

Intraoperative Floppy Iris Syndrome (IFIS) and Eye Surgery

All alpha-1 blockers, including Alfuzosin XR, are associated with Intraoperative Floppy Iris Syndrome (IFIS) during cataract surgery. This is a variant of small pupil syndrome where the iris becomes floppy and difficult to manage during the operation. If you are scheduled for eye surgery — now or in the future — tell your ophthalmologist that you are taking (or have taken) Alfuzosin XR. Don't crush or chew: Swallow the tablet whole. Crushing or chewing destroys the extended-release matrix, causing the full dose to release immediately — dramatically increasing blood pressure drop risk.

Avoid grapefruit: Grapefruit juice can interfere with the CYP3A4 enzyme that metabolizes alfuzosin, potentially raising drug levels in your blood.
